Clinical Study On Relationship Among Syndrome Differentiation Clssified In Tcm,Ambulatory Blood Pressure Monitoring,Doppler Echocardiography And Sex Hormones Level In Essential Hypertension

Objective : Essential hypertension(EH) is a sort of cardiovascular disease , it does great harm to people's health. Many clinical practices prove that Chinese medicine treatment for EH has its prevalence. The purpose of this subject is to investigate the relationship among Ambulatory Blood Pressure Monitoring(ABPM) , Doppler Echocardiography , Sex Hormones Level and syndrome differentiation classified in TCM of EH.Methods : 96 patients of EH were selected and classified into 4 types according to syndrome differentiation classified in TCM: the Liver-Fire exuberant syndrome, the phlegm-dampness abundant syndrome, Yin-deficiency and Yang-hyperactivity syndrome and the Yin-Yang dual deficiency syndrome. Statistics has been done in the level of sex hormone, the parameters of ABPM and Doppler Echocardiography. All the data above were compared among the four syndromes.Results : (1) Comparison of the four syndromes' parameters of ABPM: â‘  the Liver-Fire exuberant syndrome's 24h SBP and DBP were higher than other three syndromes, the difference among them was significant (P<0.05) ; the Yin-Yang dual deficiency syndrome's 24h SBP and DBP were lower than other three syndromes, the difference among them was significant (P<0.05). â‘¡ The four syndrome's burden of 24h SBP and DBP were higher than the control group. The Liver-Fire exuberant syndrome's burden of 24h SBP and DBP were the highest (P<0.05) ; The Yin-Yang dual deficiencysyndrome's burden of 24h SBP and DBP were the lowest (P<0.05). ? Regarding the descendent of the blood pressure during night time, The Yin-Yang dual deficiency syndrome's was the highest and the phlegm-dampness abundant syndrome's was the lowest.The difference among them was significant (P<0.05).(2) Comparison of the four syndromes' data of Doppler Echocardiography: ? The four syndromes' thickness of IVS and LVPW were higher than the control group, the phlegm-dampness abundant syndrome's was the highest (PO.05). ? The EF and FS of the phlegm-dampness abundant syndrome and the Yin-Yang dual deficiency syndrome werehigher than the control group(P>0.05). ?The E and E/A of the Yin-Yang dual deficiency syndrome were the highest(P<0.05) ; the Liver-Fire exuberant syndrome's were the lowest ( PO.05 ) .(3) Comparison of the four syndromes'data of adipose : ? Only the Yin-Yang dual deficiency syndrome's TG was normal, other three syndromes' TG were all higher than normal; the phlegm-dampness abundant syndrome's was the highest(P<0.05). ? As to CHand LDL, Yin-deficiency and Yang-hyperactivity syndrome was the highest and the Liver-Fire exuberant was the lowest.? HDL of the Yin-Yang dual deficiency syndrome was the highest and the Liver-Fire exuberant syndrome's was the lowest (P<0.05).(4) Comparison of the four syndromes' sex hormones : ? Male patients of EH: estradiol: The Yin-Yang dual deficiency syndrome's was the highest and the phlegm-dampness abundant type was the lowest. Testosterone: the Yin-Yang dualdeficiency syndrome's was the highest and the phlegm-dampness abundant syndrome was the lowest. ? In female patients of EH, the Yin-Yang dual deficiency syndrome's estradiolwas the highest and the phlegm-dampness abundant syndrome was the lowest.Yin-deficiency and Yang-hyperactivity syndrome's testosterone was the highest; theLiver-Fire exuberant syndrome's progesterone was the highest ( P<0.05 ) .Conclusion : (1) ABPM and Doppler Echocardiography ,which are importantexaminations for EH, are useful to syndrome differentiation classified in TCM of EH. (2) Sex Hormone and adipose are also helpful to syndrome differentiation classified in TCM ofEH.
