Objective : Nonalcoholic fatty liver disease(NAFLD) is a clinical syndrome characterized by excessive fat storing in liver and hepatic cellular degeneration, without alcohol over ingestion. NAFLD refers to a wide spectrum of liver damage, ranging from simple steatosis to steatohepatitis, advanced fibrosis, and cirrhosis. The incidence of NAFLD is increasing and NAFLD patients are prone to liver sclerosis or liver failure. Our aim is not only to establish a rat model of nonalcoholic steatohepatitis with fibrosis by feeding of a fat-rich diet, but also to study the preventive effect of compound traditional Chinese medicine in nonalcoholic steatohepatitis with fibrosis. Methods: SD rats were divided randomly into four groups: control group(n=8) was fed with normal diet;model group(n=14), low dose group(n=8) and high dose group(n=8) were fed with a fat-rich diet. After 16 weeks, low dose group and high dose group were perfused with Chinese herbs for 12 weeks. Meanwhile, model group and control group were perfused with NS. The serum levels of total protein(TP), albumin(ALB), alanine aminotransferase(ALT), aspartate aminotransferase(AST), total cholesterol (TC), hyaluronic acid(HA), laninin (LN), PC III and C IV were measured. The degrees of hepatic steatosis, inflammation, and fibrosis were observed with HE and V-G stains. We also observed the positive express degree of cytokine TGF- β1 with immunohistochemistry.Results: All rats of model group developed whole hepatic lobules steatosis mixed with inflammatory cell infiltration and necrosis. Central veins peripheral fibrosis and perisinosodial fibrosis also can be observed in model group with V-G stain. The levels of steatosis, steatohepatitis and fibrosis were significantly lower in low dose group and high dose group than that in model group. The levels of ALT, AST, TC, HA, LN, PC IIIand C IV in serum were significantly higher in model group than that in control group(.P<0.05). Compared with model group, low dose group and high dose group had significantly lower levels of ALT, AST, TC, HA, LN, PCIII and C IV (P<0.05). The serum levels of HA, LN, PCIII and C IV had direct correlation with fibrosis. Compared with control group, model group had significantly higher levels of cytokine TGF- Pi's positive express degree(P<0.05). High dose group was significantly lower than model group(P<0.05). There were no significantly different between model group and low dose group(P>0.05).Conclusion: We established the rat model of nonalcoholic steatohepatitis with fibrosis successfully. Compound traditional Chinese medicine decreased the levels of lipid peroxide products and the dilivery of cytokine TGF- P i, which can promoted the proliferation of hepatic stellate cells and the abnormal deposition of extracelluar matrix. Compound traditional Chinese medicine not only can therapy nonalcoholic adiposis hepatica, hyperlipoidemia and protect hepatocyte from injury, but also can prevent the emergence of nonalcoholic steatohepatitis with fibrosis.
