Objective To study the relationship between pulse pressure(PP) and coronary artery diseaseC CAD, including acute myocardial infarction and unstable angina pectoris) in essential hypertensive patients, and pulse pressure' s significance on coronary artery disease treatment and primaryprevention. Methods according to coronary artery disease diagnosis standard criterion , to divide 661 essential hypertensive patients into two groups, 184( 27.8%)patients had CAD, and 477 (72.2%) patients had no CAD, and measure interrelated materials of two groups, including systolic blood pressure, diastolic blood pressure, pulse pressure mean artery presses heart rate BMI (Body mass index), duration of hypertension, hyperlipemia, diabetes smoking drinking and so on, Logistic regression analysis can be used to control the interference of confused factors, to show the relationship between PP and CAD . Results Logistic multivariate regression analysis show that PP was strongly positive correlated with CAD. Every 5mmHg increment in PP, OR was 4.2, 95%CI: 3.36-5.26,. diastolic blood pressure was negative correlated with CAD, every 5mmHg increment in diastolic blood pressure, OR was 0.53, 95CI: 0.41-0.68. Systolic blood pressure was positive correlated with CAD, every 5mmHg increment in Systolic blood pressure, OR was 1.59, 95%CI: 1.46-1.76. Confused factors such as drinking hyperlipemia, diabetes duration of hypertension heart rate BMI have no statistic significance . Conclusion With the increment in pp of EH patients , the incidence of CAD increase accordingly. With the increment in systolic blood pressure, the incidence of CAD decrease accordingly. It suggest that therapy principle in EH patients is not only to decrease BP into objective target level, but also to provide comprehensive interference. PP may be conductive to the primary prevention of CAD and provide clinic basis in selection of anti -hypertension drugs.
