Objective: To explore the relationship of blood pressure indices and coronary artery lesions in elder hypertension person.Methods: 189 cases who were suspected to coronary heart disease were divided into two groups:observe group (135 cases with essential hypertension) and control group (54 cases with normal blood pressure). Coronary angiography (CAG) was performed in all cases. The coronary disease score(CAS) was estimated from the result of CAG. We recorded the sex, age, history of smoking and 2-diabetes, measursed brachial blood pressure ( include PP, SBP, DBP and PPI ), Height, Weight, TC, TG, HDL-L, LDL-L and calculated BMI, PP, PPL All factors including blood pressure indices and CAS were compared between the two groups. Correlations between the focters and the coronary artery lesions were assessed by simple correlation coefficients .Whether the PPI is an independent predictor of the presence and quantity of the coronary artery lesions was assessed by Multivariable Logistic and linear Regression.Results : Hypertensive subjects had higher BMI, TC (P < 0.05),components of blood pressure and CAS(P<0.01). In observe group PPI, BMI, TC, and Sex had significantly relation with presence of abnormal CAG in a Multivariable Logistic Regression, and PPI was the most significant factor (OR=18.870). In the Multivariable linear Regression PPI (b'=0.787, P< 0.001), age (b'= 0.356, P<0.01) , TC(b'=0.160, P<0.05), history of 2-diabetes (b'= 0.140, P<0.05) and smoking(b'=0.150, P < 0.05) were was associated with CAS after adjustment for sex, BMI, SBP and PP. In the group with DBP≤75mmHg, PPI (b'=0.348, P<0.001) and age were significantly correlated to CAS,comparing with that PP (b'=0.670, P< 0.001) was the most important effect factor in DBP≥85 mmHg.Conclusions: The elder hypertensive subjects had higher SBP. PP. PPI ,ratio of abnormal CAG and more severe coronary artery lesions. In hypertensive subjects PPI was an important and independent predictors of the presence and quantity of the coronary artery lesions after adjustment for age, sex, BMI, TC, history of smoking and 2-diabetes, especially in lowering DBP subjects (DBP≤75mmHg), but when DBP≥85 mmHg PP seemed the better.
