[Objective] To investigate the experience of surgical management of pulmonary sclerosing hemangioma, and summarize the pathohistological feature through literature review, with the expectation of better clinical instruction.[Methods] From 1997 to 2005, 15 cases PSH were treated surgically. There were 4 male and 11 female patient. All lesions were solitary and unilateral. 3 cases were treated with video assisted thoracoscopic surgery.[Results] 4 cases were misdiagnosed as lung cancer, which resultsin lobectomy with lymphnode mapping. Of the four misdiagnosed, 3 cases have no intraoperative frozen section. One frozen section was misdiagnosed as pulmonary carcinoid. All cases were no postoperative morbidity and mortality. No recurrence of PSH was observed in any of the patients during the following-up surveys.[Conclusions] Preoperative diagnosis of PSH is relatively few, and PSH is easily misdiagnosed as lung cancer. CT scan (enhancement necessary) and puncturation guided by CT are considered well worth for PSH diagnosis. Operation with the examination of intraoperative frozen section is actively recommended. PSH is drived from respiratory epithelium(type- II pneumonocytes and/or Clara cells).
