Surgical Anatomy And Early Clinical Application Of Endoscopic Anterior Cervical Operation

Objective: To investigate anatomic parameters of anterior cervical appliedanatomy so as to provide anatomic clues for endoscopic anterior cervical operation (EACO) and discuss the feasibility of EACO and evaluate early clinical outcome of the EACO.Methods:1.Anatomical Measurement: Anatomic data that are clinically significant were obtained by measuring the anterior cervical specimens from 25 normal Chinese cadavers, and anatomic observation was conducted in the neighboring structure from 25 cadaveric specimens.2.Surgical Model: With the MED system, we performed 14endoscopic anterior cervical discectomy on the C1-7 cervical spine of 7 cadaveric specimens. A 2cm paramidline right incision was made and a working tube of 18mm outer diameter was inserted through the space between the neurovascular sheath laterally and the visceral sheath medially and fixed to the operating table through a flexible arm. Then the endoscope was mounted to the working tube. Discectomy and curettement of posterior vertebral osteophyte was made under the help of the endoscope.3.Clinlic Outcome: 26 patients obtained endoscopic cervical discectomy and interbody fusionResults:1. Anatomical Measurement: (1) The internal laryngeal branch of the superior laryngeal nerve can be divided into three segments. The lengths of there segments were 24.2±4.2mm,6.97± 1.26mm and 13.40±1.65mm respectively . (2) The ST ran from upperlateral to lower medial.
