The Application Value Of CT Imaging In Judging The Viability Of Hepatic Cystic Hydatid.

OBJECTION In order to provide theoric foundation to judging viability of hepatic cystic hydatidosis by using CT (Computed Tomography) pre-operatively,Initial approach is set to evaluate the value of CT techology in judging the viability of hepatic cystic hydatid .METHOD Total 40 patients with 42 hepatic cysts hydatid resected by"sub-adventitial cystectomy(no daughter -cyst)confirmed pathologically is selected post-operatively.the CT value of mother cyst content is measured and imageologic change of endocyst is recorded preoperatively, Three kinds characters(liquid jelly-like solidation) of content of cystic is recorded respectively, postoperatively Pathologic change of Endocyst (detached form outercyst .rupture and collapse) is observed .HE-stained slice is made to observed germinal membrane and cuticular membrane which determined the viability of mother cystic .Materies excised surgically from hydatid cyst of the liver was examined with an light microscope to determine how much protoscolex is living which can demonstrate the viability of mother cyst.RESULT The pathologic result post-operatively of this group show :the rate of three kinds (liquid,jelly-like,solidation )of content is followed respectively :28.5%,38.1%,33.3%. The variability of the CT value of three kinds of cystic content (liquid jelly-like solidation) is significant(P=0.00)according to statistic, the mean of CT value respectively is 4.75HU,19.17HU,48.73HU. When the CT value represent liquid state, the number of alive cyst demonstrated by protoscolex is higher than the group which present jelly-like content P<0.05.The viability of mother cyst demonstrated by protoscolex is decrease following the increase of CT value of cystic content . No viable protoscoleces exit in 14 consolidated cyst ; the endocyst is aggravated following it,s peel off and subside , while the viability of the hepatic hydatid cyst is diminished following histological damage of endocyst.CONCLUSION According to CT value and imageologic change of endocyst of hepatic hydatid cyst ,the viability of cyst can be judged by CT technology in vito.Based on the character of cystic content and pathologic change of endocyst, CT technology play a crucial role in judging viability of hepatic hydatid cyst .
