Background and Objective: Acute coronary syndrome(ACS), which includes acute myocardial infarction(AMI) and unstable angina pectoris(UAP), is a special group of coronary atherosclerotic disease. Coronary atherosclerosis is a kind of disease that is involved in dyslipidemia, plaque formation and rupture, platelet activation and thrombosis, etc. Plaque rupture and platelet activation ultimately result in thrombosis, which leads to complete or incomplete occlusion of coronary artery. It has been proved that platelets play an important role in the pathogenesis of ACS. When platelets are activated by some pathological factors, the expression of P-selectin will be increased significantly. P-selectin is a component of the platelet membrane and endothelial intracellular storage organelles, and its appearance on the surface (defined by flow cytometry) implies platelet activation, under which condition it mediates cell/platelet adhesion. A soluble form of P-selectin, detectable by enzyme-linked immunosorbent assay (ELISA), present in the blood implies increased platelet activation. High levels of soluble P-selectin are found in cardiovascular disease, smoking, diabetes mellitus, etc. Preliminary data suggest that high levels of P-selectin predict adverse outcome. Its role in mediating intercellular adhesion has prompted work on intervening with this process as a new therapy. Because soluble P-selectin is a platelet product, we may expect antiplatelet therapy to have effects on it. Many studies about effects of clopidogrel on P-selectin have focused on membrane P-selectin. However, few studies have looked at effects of clopidogrel on soluble P-selectin.
