Operative Procedure Of Z-open-door Expansive Laminoplasty For Cervical Spondylotic Myelopathy

Objective: To investigate the effect and security for treatment of CSM by expansive Z-open-door laminoplasty.Methods: 56 cases of CSM were operated upon with expansive Z-open-door laminoplasty.The effect was evaluated by 17 scroe of JOA and the image before and after operation.SEP was being monitored in all of cases. Result: 27 caese were followed up in an average of 13.6 months. The JOA scores of all cases are improved 58.4%. Image show: three of 51 cases of the front to line type changed into the straight line type, all of 5 cases of the straight line type still keeped the straight line type, and none changed into the back to line type after operation. There were significant statistical discrepancy between the range of motion of cervical vertebra and pain. SEP showed no change in 49 cases before and during and after operation,and showed significant comeback in 7 cases.Conclusion: Expansive Z-open-door laminoplasty for CSM can decompress thoroughly and expansive laminoplasty adequately. It accordingly prevents to close door again and the disc space with kyphosis,and keeps structure of stability of cervical vertebrae furthest.So it is a simple and safe method.
