Recent studies suggest that the risk factors for marginal bone loss around implants are smoking, infection, mechanical loading, bone quality and metabolic diseases. In the two-stage surgical procedure, mechanical loading is absent between stage I and stage II of surgery. What's more, scholars generally regard that there is no marginal bone loss around the implants during the healing period. In fact, the marginal bone loss does really happen in this period, which may due to the potential individual factors. Interleukin-1 and tumor necrosis factor-a are both strong bone absorptive stimulators. Interleukin-1 receptor antagonist (IL-1ra) is the potent mediator of inflammation, which binds to IL-1 receptors competitively and inhibits the biological action of IL-1. The gene coding for IL-1ra (IL-1RN) has a polymorphic site of 86 base pair variable number tandem repeat (VNTR) within the intron 2,which can alter the IL-1ra expression.
