| Objective: To investigate the longterm therapeutic effect of optimized cardiac resynchronization therapy directed by ultrasound in chronic heart failureMethods: Two chronic heart failure patients with complete left bundle branch block were implanted pacemaker for cardiac resynchronization therapy, based on sufficient drug therapy. They still took drug therapy after implantation of pacemaker. 1 month after the implantation of pacemaker, the pacemakers were optimized by the direction of cardiac ultrasound, rational A-V delay and V-V delay was installed. 1 year after the implantation of pacemaker, the patients was followed up, the symptom, exercise tolerance and NYHA cardiac functional grading were evaluted, ECG and UCG were reviewed.Result: The symptoms of patients were relieved, the exercisetolerance were improved, the NYHA cardiac functional grading were decreased to II grade. The QRS durations were decurtated. The left hearts were deflated, the LVEF were increased, the depressures of pulmonary artery were decreased.Conclusions: optimized cardiac resynchronization therapy directed by ultrasound in chronic heart failure can ameliorate the asynchronization of heart, relieve clinic symptom, improve exercise tolerance and cardiac functional, reverse the reconstitution of ventricle. |
