| Objective To introduce the technique of internal posterior transpedicular fixation for treatment of thoracolumbar spinal fracture by paraspinal approach and compare with the preliminary clinical outcomes of the treatment of thoracolumbar spinal fracture with traditional internal posterior transpedicular fixation by midline approach.Methods Internal posterior transpedicular fixation by paraspinal approach was used to treat forty cases of thoracolumbar spinal fracture. Perioperative parameter and the index of image, the preoperative and postoperative cross-sectional area of multifidus muscle measured by MRI were compared with the treatment of traditional internal posterior transpedicular fixation by midline approach in other forty cases.Results The consumed time of operation and postoperative analgesic injection time in the paraspinal approach group and the midline approachgroup made no significant difference (P>0. 05), but the injury of paraspinal muscles, bleeding of operation, drain of postoperation, pain score of postoperation spending time of hospitalization were all significantly different (P<0.05) Each group compared to itself between preoperation and postoperation,the vertebral height,the height of intercertebral disk,Cobb' s angle were all significantly different (P<0. 05). The preoperative and half a year postoperative cross-section area of multifidus muscle measured by MRI in the paraspinal approach group and the midline approach group made significant difference (P<0. 05).Conclusions The technique of internal posterior transpedicular fixation for treatment of thoracolumbar spinal fracture by paraspinal approach has the advantages of simple manipulation, safety, less bleeding,light pain, quickly recovery and short hospotalizaton time. |
