Background: Colorectal cancer is the most familiar malignant carcinoma, ln the world, colorectal cancer ranks 3 in the incidence of maligant of malignant carcinoma. In our country, it' s attack rate has raised from 6 to 5. Every year,there are about 1,023,152 new cases of colorectal cancer all over the world, about 40% of them are advanced cases, and about 50% of the patients who had received radical correction would come to recurrance or metastasis. The prognosis of advanced refractory colorectal cancer is bad. The apprearance of new drugs don' t prolong the survival of these patients, and their effective power have not exceeded 50%. Treatment of TCM pay attention to the correlation of whole and local, unify "differentiation of disease" and "differentiation of symptoms" Treatment of TCM can steady the tamour, prolong the survival and improve the quality of life(QOL).Objective: To observe the therapeutic effect of "Remove Blood Stasis and Relieve Toxicity" of Traditional Chinese Medicine(TCM) in the Treatment of advanced colorectal cancer.Methods: 46 colorectal cancer patients in the stage ofⅢorⅣwere allocated into two groups, 24 received TCM (groupⅠ)or combined treatment of TCM and 20 received Western medicine respectively(groupⅡ). But 4 ablate from groupⅠat the halfway, and 2 for groupⅡ. Project for groupⅠ: with combination of syndrome differentiation and disease differentiation, Pingxiao capsules was used according to disease differentiation, and different dicoctions were determined from three patterns: Accumulation of Dampness and Heatness of Colon, Stagnation of Blood and Toxic Materials Stasis of Colon, Defficiency of both Spleen and kidney according to syndrome differentiation, and the period of treatment was three courses, 1 mouth per course. Project for groupⅡ: the treatment of TCM is the same as groupⅠ, chemothrapy adopted as either FOLFOX4/FOLFOX6/FOLFOX7, they were repeated every 14 days, 6 times. Evaluate the curative effect of TCM by tumor response rate, weight change, functional status improvement, suervival.Result:1. Tumor response rate (CR+PR) were 5% of groupⅠ,25% of groupⅡ;the disease control rate were 65% of groupⅠ,80% of groupⅡ.There was statistical significance between two groups on tumor response rate, but was no statistical significance between two groups on the disease control rate.2. Body weight of the patients in two groups increased compared which of prior-treatment, there was statistical significance between prior-treatment and post-treatment. The value of CEA and CA199 decreased after treatment, there were statistical significance between prior-treatment and post-treatment. However, there were no statistical significance between two groups in body weight, CEA and CA199 of post-treatment.3. There were statistical significance between prior-treatment and post-treatment in symptome improvement, Karnorfsy score and QOL. The result of QOL show that treatment integrating TCM and Western medicine can improve the QOL of advanced colorectal cancer in physical, emotional, additional concerning domain better than treatment of TCM.4. The median survival time of two group were groupⅠ332 days (11.6 months), groupⅡ478 days (13.2 months). survival rate of 0.5-year, 1-year, 1.5-year was 80%, 30%, 0% respectively with groupⅠ; and groupⅡwas 95%, 55%, 43% respectively. There were statistical significance between two groups.Conclusion:The treatment of TCM can extends the overall survival, steadying tumours, improve the QOL of colorectal cancer advanced patients.The treatment of integrating TCM and Western medicine can do beteer in contractting the gross tumor volume, improving functional status and QOL, prolonging suvival than treatment of TCM.
