Background:Colorectal cancer (Colorectal cancer, CRC) is one of the most common malignant tumors of clinical, now worldwide, the incidence rate of colorectal cancer in the first three. With the Westernization of diet in China, our CRC morbidity and mortality significantly increased in the last 30 years, is a common malignant cause of death in four. The study reported that the incidence of colorectal cancer after the age of 60 showed a sharp upward trend, reached a peak after 80 years of age. With the population aging, elderly patients with colorectal cancer is increasing in absolute terms. However, due to physical function in elderly patients, social, family and economic constraints and other factors, a considerable number of elderly patients diagnosed at advanced stage and lost the chance of operation, while more than half of patients relapse after treatment and metastasis. Therefore, for the elderlyâ…¢, IV colorectal cancer patients to choose the safety, tolerability and efficiency of chemotherapy and efficiency of Chinese medicine treatment attenuated oncology community has become an important issue.Purpose:To evaluate response, survival and tolerability in patients with advanced-stage(â…¢/â…£) elderly CRC receiving "Remove Blood Stasis and Relieve Toxicity" of TCM therapy, and to observe the effect of reducing toxicity and increasing efficacy by combine with chemotherapy. To search the optimal treatment strategy for elderly patients with advanced CRC.Patients and Method:The retrospective study was performed on 65 cases of senile III, IV colorectal cancer who was seen at the Department of Clinical Oncology, in the First Affiliated Hospital of Guangzhou University of Traditional Chinese Medicine Guangzhou University in the last 5 years (group I), received combined treatment of TCM and Western medicine, compared with 26 patients who was seen at the Guangdong Armed Police Hospital (groupâ…¡). The primary end point wais survival, the other end points included time to disease progression (TTP), tumor response rate, weight change, CEA change ect. Suvival was calculated by the method of Kaplan-Meier and compared by log rank test.Multivariate analysis was Calculated by the Cox proportional hazard model.Result:The median survival time of two groups were groupâ… 1000 days, groupâ…¡755 days, with a 1-year survival rate of 95.4%,96.4%, there were no statistical significance between two groups.TTP of two groups were groupâ… 225 days, groupâ…¡173 days.The Cox regression analysis involve 14 covariates:treatment, sex,scores of symptoms, intestinal obstruction, surgical, chemotherapy, molecular targeted therapy, TNM stage, location of tumour, liver metastasis, distant lymph node metastasis, the average HGB level before treatment, Carcinoembryonic antigen(CEA), KPS before treatment. The result showed that independent prognosis included treatment, scores of symptoms, surgical, chemotherapy, distant lymph node metastasis. Conclusions:The treatment of TCM can extends the all survival of the elderly patients with advanced CRC in a poor condition.The treatment of TCM can be recommended as a feasible treatment strategy for elderly patients with advanced CRC, and "Remove Blood Stasis and Relieve Toxicity" can be considered as a primary therapeutic principle of TCM.
