Significance Of FHIT,P53 And P16 In Cervical Diseases

Objective: To explore the function, significance and interaction of Fragile histidine triad (FHIT),P53 and P16 protein in cervicitis,cervical intraepithelial neoplasia (CIN) and cervical cancer (CC) tissue.Method: Tissue microarray(TMA) technology and immunohistochemical method was used to measure the expression of FHIT, P53 and P16 in 17 cervictis tissue, 65 CIN tissue and 98CC tissue. The statistical software-SPSS13. 0 was applied to analyze the resuts by chi-square test, Spearman's correlation test and rank sum test.Result:①The expression rate of FHIT in cervicitis was 100 % but the descending rate or the rate of deletion in CINⅠ,Ⅱ,Ⅲwas 25. 0%, 42. 9%and 66. 0% respectively; And in CC the loss rate was 61. 2 %; There was no significant difference between the group of squamous cell carcinoma (64. 0%) and the group of adeno carcinoma (50. 0%) (P>0. 05). In different clinical stages the rate of deletion was 56. 1%, 74. 2 % and 50. 0 % respectively and that in different histological grade G1, G2, G3 was 78. 6%, 54. 3 % and 72. 4%. They didn't have apparent difference(P>0. 05).②There was no positive expression of P53 in control group; The positive rate of P53 in CINⅠ,Ⅱ,Ⅲwas 11. 1%,28. 6% and 46. 7% respectively; And in CC group the rate was 71. 4%. Besides it increased with the increase of clinical stage, the decrease of differentiation degree and lymphaden metastasis. The difference of P53 expression was obvious between CINⅠand CINⅢ(P<0. 05), as well as control group and CC group,CIN group and CCgroup (P<0. 01). But there were no significant difference between CINⅠand CINⅡ,CINⅡand CINⅢ,clinical stageⅠand stageⅡ(P<0. 05). The expression was much higher in squamous cell carcinoma (75. 3%) than that in adenosquamous carcinoma (33. 3%) (P<0. 01). And the expression was significantly lower in=40 yesrs old group(57. 1% )than that in>40 yesrs old group(82. 1%) (P<0. 01). Moreover the expression of P53 had no correlation with histological grade and lymphaden metastasis.③The positive rate of P16 in control group, CINⅠ,Ⅱ,Ⅲand CC group was 35. 3%, 47. 2%, 64. 3%, 80%, 91. 8% respectively. The difference has statistical significance between control group and CINⅢ(P<0. 05) as well as the distinction between the group of CINⅠand CINⅢ. But the distinction between other every two groups had no statistical significance (P>0. 05). The expression of P16 decreased with increasing of differentiation: well differentiated 100 %, moderately differentiated 91. 1%, poorly differentiated 70 %. Further the poorly differentiated group had significant difference of the P16 expression compared with the well differentiated group and the moderately differentiated group (P<0. 01), however the difference betweenthe well differentiated group and the moderately differentiated had no statistical significance. Its positive expression in squamous cell carcinoma group (94. 4%) was much higher than that in adenosquamous carcinoma group(66. 7%)(P<0. 01). The ratio of its overexpression also had obvious difference between lymph node metastasis group (100%)and without lymph node metastasis group(85. 3%) (P<0. 05). But its overexpression had no correlation with age and clinical stage.④Analyzed by the Spearman's correlation test the resuts showed that in CIN group there was no correlation among the three index: FHIT, P53 and P16.⑤In CC group there was positive correlation between FHIT and P16 (R=0. 743, P=0. 000) while FHIT and P53 had no correlation.Conclusion:①The deletion of FHIT and overexpression of P53 occur frequently in CC so it can be the index to screening high-risk group. It is aslo used as prognostic indicator in CC.②The high espression of P16 protein is the early manifestation in CC while the overexpression of P53 is the late manifestation. Therefore we can screening highrisk patients by monitoring the level of P16 and p53 protein. Then the clinican can take effective measures and give them early therapy. So the cure and survival rate will be improved. FHIT and P53 are closely related to cervical carcinogenesis and its development that can be theaccessorial index to estimate the prognosis.
