Artery Perfusion And Screw Fixtation-support In Combination With Syndrome Differentiation Of Chinese Medicine For Osteonecrosis Of Femoral Head: A Preliminary Clinical Study

Purpose: To observe the preliminary effect of the treatment of artery perfusion and screw fixation-support combined with Chinese medicine for pericollapse stage ONFH.Materials and methods: The patients were all from the first Affiliated Hospital of the Guangzhou University of Traditional Chinese Medicine who had suffered from ONFH. According to age, sex, etiological factor, and the size of the femoral head necrosis lesion, the groups were matched to" Group A (artery perfusion+support+Chinese medicine) :13 cases/18 hips, stageⅡ-13hips(ⅡA-2hips,ⅡB-5hips,ⅡC-6hips), stageⅢA-5hips; Group B(suppor+Chinese medicine):20cases/25hips,stageⅡ-17hips(ⅡA-1hips,ⅡB-7hips,ⅡC-9hips), stageⅢA-8hips; Group C (artery perfusion+Chinese medicine) 10 cases/15 hips, stageⅡ-13hips (ⅡA-4hips,ⅡB-6hips,ⅡC-3hips),stageⅢA-2hips.The follow-up time were 3,6,12,18months.All the three groups were evaluated on clinical and imaging. By comparing the difference of Harris scores , the rate of pain relief and the imaging good-to-excellent rate of each group ,the clinical and imaging evaluation were observed and analysed. The results were statistically treated with Software SPSS.Results: 1.clinical evaluation result:①the scores of clinical effect of the three groups all got enhanced, as to Group A, average enhancing score was 9.67, to B was 8.48, to C was 4.27. Analyzing by one simple T-test, the clinical effect of Group A and B both surpassed Group C, P<0.05, the effect of Group A was a little bit better than Group B, P>0.05, there was no significant difference between them.②the pain symptoms of the three groups all got relief disparitedly ,as to Group A, the rate of pain relief was 94.4 %, To B was 84%, To C was 80%. In Ridit analysis, the difference of the rate of pain relief was significant between Group A and Group B, C.2.imaging evaluation: the overall good-to-excellent rate of the three groups :as to Group A was 83.3%, to B was 80%, to C was 53.3%. In Ridit analysis, the imagine evaluation of Group A and B both surpassed Group C, P<0.05,but Group A and B has no statistical significance.Conclusions. It has been certain initial evaluation that as to treat ONFH, perfusion combined with screw fixation-support is better than simple operation on pain relief, combined with the syndrome of Chinese medicine are able to relief pain, improve microcirculation, promote rehabilitation, and prohibit collapse. The treatment is applicable to the pain relief of the pericollapse stage ONFH, but it should be further studied on whether it is better than simple support on prohibiting or diminishing collapse.
