ObjectiveTo investigate the significance of hypoxia-inducible factor-1a(HIF-1α) and its correlation with angiogenesis in precancerosis and cervival cancer by detecting the expression of HIF-1αand vascular endothelial growth factor(VEGF) in cervical intraepithelial neoplasia(CIN) and cervival cancer, and analyzing the association between the two factors.MethodsImmunohistochemical methods were used to detect the protein expression of HIF-1αand VEGF in paraffin-embedded specimens of 79 cervical cancer,33 CINⅢ,28 CINⅡ,29 CINⅠand 20 normal cervical tissue.The correlation of the two factors with the type of cervical lesions and clinical feature of cervical cancer was evaluated by chi-square test,and the relationship of the two factors was analyzde by Spearman rank correlation test.Results1.(1)The positive percentage of expressions of HIF-1αin normal cervical tissue,CINⅠ-Ⅱ, CINⅢand cervical cancer were 5.0%,21.1%,54.5% and78.5% respectively. The positive percent of HIF-1αin CINⅢwas significantly higher than CINⅠ-Ⅱand normal cervical tissues (p<0.05),and which in cervical cancer was significantly higher than CINⅢ(p<0.05).(2) The positive percentage of expressions of HIF-1αin different histological grading cervical cancer was 47.1% in high grading,77.8% in middle grading and 100.0% in low grading.The positive percent of HIF-1αin moderate differentiation was significantly higher than well differentiation(p<0.05),and which in poor differentiation was higher than moderate differentiation(p<0.05). The positive percentage of expressions of HIF-1αin different stage cervical cancer was stageⅠ69.0%,stageⅡ76.0% and stageⅢ92.0%.There was no difference between stageⅠand stageⅡ, stageⅡand stageⅢ,but the percentage of HIF-1αexpression in stageⅢwas significantly higher than that in stageⅠ(p<0.05).2. 1)The positive percentage of expressions of VEGF in normal cervical tissue, CINⅠ-Ⅱ, CINⅢand cervical cancer were 10.0%,38.6%,60.6% and81.0% respectively. The positive percent of VEGF in CINⅠ-Ⅱwas significantly higher than normal cervical tissues(p<0.05),which in CINⅢwas significantly higher than CINⅠ-Ⅱ(p<0.05),and in cervical cancer was significantly higher than CINⅢ(p<0.05).2) The positive percentage of expressions of VEGF in different histological grading cervical cancer was 52.9% in well grading,86.1% in moderate grading and 92.3% in poor grading respectively.The positive percent of VEGF in moderte differentiation was significantly higher than well differentiation(p<0.05),but there was no significance difference between poor and moderate differentiation(p>0.05). The positive percentage of expressions of VEGF in different stage cervical cancer were stageⅠ69.0%,stageⅡ76.0% and stageⅢ92.0%. The positive percent of VEGF in stageⅡwas significantly higher than stageⅠ (p<0.05),but there was no significance difference between stageⅡand stageⅢ(p>0.05).3.The expression of HIF-1αhad positive correlation with the VEGF by Spearman rank correlation test (r=.0632, p<0.05).Conclusion1. The positive expression rate of protein of HIF-1αand VEGF showed tendency of ascensus with the progression of cervical leisions.2. The protein of HIF-1αand VEGF were overexpressed in cervical cancer,further more they were positive related with clinical stages and negative related with histological grading.3. The expression of HIF-1αhad positive correlation with the VEGF in cervical lesions. HIF-1αmay nduce angiogenesis by up-regulating the protein product of its target gene--VEGF, so that to promote the progression of cervical precancerous leision and the infiltration of cercical cancer.
