| OBJECTIVE: To access the possibility and practicability of MEWS and to compare the performance with RTS on trauma patients in emergency department.METHODS: A large teaching hospital in china was selected as study setting. The patients who were hospitalized to emergency department with trauma were enrolled in study in 4 months. All of 213 patients were evaluated with MEWS and RTS instantly when they were sent to emergency department. The severity of trauma, shunt and prognosis within 1 month were followed-up and their relationship, correlation with MEWS were statistical analyzed after dividing into groups retrospectively. Moreover, the performance between MEWS and RTS was compared to find the clinic value of MEWS.RESAULTS:1. MEWS has positive correlation with the severity of trauma,ICU hospitalization and mortality. Its correlation coefficient is 0.635,0.732,0.337 respectively.2. The MEWS with AUROCC 0.932 and the best cutoff score 4, has accurate discrimination on ICU hospitalization of emergency adult trauma patients. The RTS with AUROCC 0.859 and the best cutoff score 10 , has moderate discrimination on ICU hospitalization .there is significant statistic difference (P<0.01 ) of AUROCC between them .It shows that the evaluation of MEWS on ICU hospitalization has more accurate than RTS.3. The RTS with AUROCC 0.938 and the best cutoff score 9, has accurate discrimination on mortality of emergency adult trauma patients. The MEWS with AUROCC 0.852 and the best cutoff score 6 , has moderate discrimination on mortality .there is statistic difference (P<0.05) of AUROCC between them . It shows that the evaluation of RTS on mortality has more accurate than MEWS.CONCLUSIONS: MEWS can be applied to evaluate quickly the emergency trauma patients, especial in distribution. |
