Objective to approach the advantageous and disadvantageous oftwo different operative routes in treatments of supracondylar humerusfractures in children.Methods a retrospective review was made of 23 patients ofsupracondylar humerus fracture who undergone surgical treatment fromjanuaty 2002 to october 2006.to compare elbow Joint function of patientswho adopt different operative route and analyse the agents, at the sametime the incidence rate of cubitus varus was statisticated and analysed inthis series of patients.P<0.05 is to be considerated having statisticalsignificance.Results All of the 23 cases got follow-up and the following-timeis from 3 to 60 months, 34.2 months in average. In the group of outermargin of elbow approach: mean operating time was 57.3(40~75min);mean bleeding vliume was 41.0ml(30~90ml); mean healing time was1.7months(1.5~2months); Elbow joint function after operation, excellent1 cases, fine 4 cases, middle 2 cases, bad 3 cases. The rate of finenesswas 50%; 3 cases got cubitus varus and the rate was 30%. In the group ofcombined approach: mean operating time was 67.4(50~90min); mean bleeding vliume was 44.7m1(40~100ml); mean healing time was1.75months(1.5~2months); Elbow joint function after operation, excellent4 cases, fine 6 cases, middle 3 cases, bad 0 cases. The rate of finenesswas 76.9%; 3 cases got cubitus varus and the rate was 23.1%. The elbowjoint function of the latter group was better than the former group.and theresult was sensefull (P <0.05>. Two groups had no obvious difference inthe rate of cubitus varus

0.05).Conclusions 1. Combined approach is obviously better than outermargin of elbow approach, its range of motion is bigger and functionrecover better; 2. There is no obvious relationship between operativeroute and the incidence of cubitus varus. 3. The primary complication ofsupracondylar humerus fractures in children is cubitus varus.
