| Objective To investigate the normal value of Baumann's angle and the relationship between Baumann angle and Carrying angle and its clinical significance .Additionally, Improved three mats treatment was applied to deal with supracondylar fracture of the humerus for reducing the rate of cubitus varus deformity occurrence . Methods:Study 1: Baumann angle (B angle) and carrying angle (C angle)were measured in 76 normal children (aged2~12 years). Study 2: Improved three mats treatment instead of the traditional two mats treatment was appled to prevent cubitus varus deformity in the children with supracondylar fracture of humerus. Methods:We implemented three mats treament to correct ulnar deviation in 37 children (aged2~12 years) with supracondylar fracture of humerus, camparing Baumann angles with ones of the normal elbows, we restored Baumann angles until they were statisfing. Result: Study 1 :The average value of left B angle was 70.56 5.35 and C angle 9.81 4.29 .There was significant negative correlation between Bangle and C angle (P<0.01). There was no significance between left and right B angle, the result was the same in C angle. The average value of B angle of normal elbows was 70.45 5.31 , in ill elbows, it was 87.2 6.8 before reposition, 71.35 3.25 after reposition . The C angle of the normal elbows was 10.47 4.58 , in the ill elbows it was 4.51 3.72 before reposition, 11.35 5.16 after disbanding the fixation; 10.78 4.19 during the follovving-up. Respecetively comparing Baumann angle of the normal elbows with one of the ill elbows after reposition, after disbanding fixation, and comparing the carrying angle of normal elbows with one in ill elbows after disbanding fixation, during the follovving-up, there was no significant difference(P>0.05), only one had a cubitus varus, x2 = 13.948, p<0.01.Conclusion: There was a significant negative correcation between B angle and C angle.Because ulnar deviation was the major factor causing cubitus varus complication, we performed three mats treatment against ulnar deviation, which could not only restore the normal axis of the fractured humerus, but also prevent recurrence of ulnar deviation during the fixation, then the rate of occurrence could be decreased.The chang of B angle can be used as reference to predict the complicated cubitus varus in supracondylar fracture of the humerus and three points pressing can prevent cubitus varus deformity effectively... |
