Risk Factors In Patients With Acute Ischemic Versus Hemorrhagic Stroke

Objective To estimate the constituent ratio of various risk factors for stroke and to identify risk factors for cerebral infarction (CI) versus intracerebral hemorrhage (ICH).Methods We analyzed data of patients,who were treated in the first affiliated hospital of Fijian Medical University during 1995 to 2002,to describe risk factors for stroke subtype, i.e., age, gender, hypertension.Results Data from a total of 1922 patients, consisting of 1525 patients with CI and 397 patients with ICH were enrolled. Hypertension and higher pulse pressure were the most prominent factors for overall stroke. The proportion for hypertension was 73.0% for ICH versus 76.9% for CI without significant difference.The factors significantly associated with CI as opposed to ICH were atrial fibrillation(OR =3.992) followed by obesity(OR =3.745), diabetes mellitus(OR =3.576), hyperfibrinogenemia (OR =3.004), family history of hypertension(OR =2.452), promoted serum 1ow-density lipoprotein cholesterol(LDL-C) (OR =2.396), history of stroke(OR =2.217), smoking(OR =1.897), hypertriglyceridemia(OR =1.706), increasing age (OR =1.565)and low serum apolipoprotein A(Apo A) (OR =1.497) (P﹤0.05). Hypertension (OR =0.594)and hyperglycemia by a stress response(OR =0.623) were the significant factors associating with ICH versus CI(P﹤0.05).Conclusions The factors for ischemic and hemorrhagic stroke are not the same.Hypertension is the most important risk factor for ICH.Atrial fibrillation, obesity,smoking, diabetes mellitus,hyperfibrinogenemia and family history of hypertension should be the most powerful factors associated with CI rather than ICH.
