Objective:1. To describe the level of comfort of kidney transplant recipients in special nursing care period.2. To explore the influencing factors of kidney transplant recipients'comfort.3. To evaluate the effects of progressive muscle relaxation(PMR) on relieving anxiety and depression status and promoting the level of comfort of kidney transplant recipients.Methods:1. This study developed kidney transplant recipients comfort scale based on Kolcaba's General Comfort Questionnaire(GCQ) and evaluated its'reliability,validity,feasibility and responsibility by consulting eight related experts and interviewing 100 kidney transplant recipients.2. A convenient sample of 92 kidney transplant recipients in special nursing care period in a provincial general hospital provided data. The patients were interviewed and administered a questionnaire that was composed of demographic information, kidney transplant recipients'comfort scale and related factors with comfort.3. 87 samples were divided into two groups, the experimental group and the control group. The intervention group received PMR and conventional nursing measures. The control group received conventional nursing measures only. Evaluating the effect of PMR with the differences of incidence rate of anxiety and depression by SAS, SDS and the difference of the score by the kidney transplant recipients'comfort scale between the two groups.Results:1. The internal consistency (Cronbach's alpha) for the overall scale is 0.92, and each dimension of the scale is 0.739~0.897. The intra-class correlation coefficients are all greater than 0.7 in repeated measurement reliability. Four factors are extracted through factor analysis and they can explain 74.21% of the overall covariance. The factors and their subordinate items are consistency with the original hypothesis.2. The mean score of holistic comfort of the 92 kidney transplant recipients in special nursing care period is 66.72±10.15. The mean score of each item of physical comfort, psycho-spiritual comfort, social comfort and environmental comfort are 2.69±0.95, 2.42±0.92, 2.72±0.87 and 3.18±0.67 respectively.3. The holistic comfort of the patients in intensive care period is correlated with age, domestic economic, medical care system and renal function.4. Tiredness, dry mouth and thirst, pain, transfusion , drainage tubes , body position and quality of service are the factors related to comfort.5. The differences of the incidence rate of anxiety and depression and the score of comfort between the experimental group and the control group have statistical significance after PMR intervention.Conclusion:1. The kidney transplant recipients comfort scale has good reliability, validity and responsibility. It can be applied to nursing assessment and intervention evaluation on kidney transplant recipients.2. The level of psycho-spiritual comfort of the 92 kidney transplant recipients in intensive care period is lowest, the physical and the social comfort are the next and the environmental comfort is the highest. Nurses should pay attention to mental nursing, meet physical comfort needs and promote social support.3. Nurses should Keep a good quality of service and take measures to intervene the influencing factors of comfort such as tiredness, dry mouth and thirst, pain, transfusion , drainage tubes and body position.4. Nurses should pay more attention to such patients as younger, at bad domestic economic status and at themselves'expenses, especially those who fail to rehabilitate at renal function .5. The effect of progressive muscle relaxation on relieving anxiety and depression status and promoting the level of comfort of kidney transplant recipients is significant.
