Adiponectin, an adipocyte-secreted cytoactive factor, is known to haveanti-atherogenic, anti-inflammatory, anti-diabetic and anti-intimal hyperplasia of injuredartery properties. Adiponectin has been reported to possess a protective effect against theinitiation and progression of Coronary Artery Disease (CAD). Recently, the importance ofSingle Nucleotide Polymorphisms (SNPs) of adiponectin has been widely recognized.However, the associations between the Traditional Chinese Medicine (TCM) Syndrometyping differentiation of Acute Coronary Syndrome (ACS) and the SNPs of adiponectin arestill not clear. The latter may be the molecular biology base of the former. The present studyis designed to examine the associations of two single nucleotide polymorphisms (SNPs)+45 T>G and +276 G>T of adiponectin gene with TCM syndrome-type in ACS throughtheory and clinical research.ObjectiveThe present study was designed to examine the associations of two single nucleotidepolymorphisms (SNPs) +45 T>G and +276 G>T of adiponectin gene with ACS and TCMsyndrome-type in ACS.Methods77 patients who were diagnosed as ACS by the case history of CHD, clinicalmanifestation of ACS, dynamic change of electrocardiogram, myocardial zymogram andcoronary artery angiography (CAG), were divided into 4 groups according to the SyndromeDifferentiation-typing TCM, the Heart blood stasis obstruction Syndrome group, the bothQi-Yin deficiency Syndrome group, phlegm turbidity congestion Syndrome group,heart-kidney yin vacuity Syndrome group. Eighty-two healthy volunteers (NT group) wereenrolled. Genotypes for the two SNPs in the adiponectin gene were investigated by thepolymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP). Wecompared genotype frequencies and allelic frequencies between ACS subjects and normalsubjects and among the four groups of the Syndrome Differentiation-typing TCM of ACSsubjects. Results1. No significant difference was found in age and gender constituent ratio between theACS groups and NT groups. The frequencies of SNP+45 and SNP+276 adiponectingenotypes are consistent with Hardy-Weinberg law, which indicated the samples come fromthe same group.2. No significant differences were observed in the genotype and allelic frequencies ofadiponectin gene SNP+45 between the ACS and NT groups.3. Significant differences were observed in the frequencies of SNP+276 adiponectingenotypes between the ACS and NT groups. The TT genotype frequency and the T allelicfrequency of SNP+276 adiponectin in ACS patients were lower than that of the normalcontrols.4. The patients of ACS group were divided into the sthenia syndrome group and theasthenia syndrome group according to the Syndrome Differentiation-typing TCM. Thefrequencies of SNP+45 and SNP+276 adiponectin genotypes are all consistent withHardy-Weinberg law.5. No significant differences were observed in the genotype and allelic frequencies ofadiponectin gene SNP+45 between the sthenia syndrome group and the asthenia syndromegroup.6. Significant differences were observed in the frequencies of SNP+276 adiponectingenotypes between the sthenia syndrome group and the asthenia syndrome group. The GGgenotype frequency of SNP+276 in sthenia syndrome group was higher than that of theasthenia syndrome group. The allelic frequency of the two groups has no significantdifferences.7. The patients of ACS group were divided into four groups according to theSyndrome Differentiation-typing TCM. No significant differences were observed in thegenotype and allelic frequencies of adiponectin gene SNP+45 among the four syndromegroups.8. Significant differences were observed in the frequencies of SNP+276 adiponectingenotypes among the four syndrome groups. The allelic frequency of the four groups has nosignificant differences.9. Significant differences were observed in the frequencies of SNP+276 adiponectingenotypes between the Heart blood stasis obstruction Syndrome group and heart-kidney yinvacuity Syndrome group, the both Qi-Yin deficiency Syndrome group and heart-kidney yin vacuity Syndrome group. The TT genotype frequencies of the Heart blood stasisobstruction Syndrome group and the both Qi-Yin deficiency Syndrome group are higherthan heart-kidney yin vacuity Syndrome group.Conclusion1. The SNP+276 adiponectin genotypes were associated with the presence of ACS.The G allele of SNP+276 is a geno-factor while the T allele is a protective factor.2. The SNP +45 in adiponectin gene were not associated with the presence of ACS.3. The SNP+276 in adiponectin gene were associated with the SyndromeDifferentiation-typing TCM of ACS and the sthenia syndrome group and the astheniasyndrome group of ACS.4. The SNP+45 in adiponectin gene were not associated with the SyndromeDifferentiation-typing TCM of ACS.
