Objective To compare BIS with AEPex in research of clonidine as premedication in propofol-remifentanil anaesthesia. Methods Forty-eight ASA I~II patients, aged 20~60yr ,undergoing laparoscopic cholecystectomy were randomly divided into two groups(n=24).Clonidine3μg/kg was intravenously given in group II 15 min before induction of anaesthesia .Bispectral index,Auditory evoked potential index,BP,HR and SPO2 were monitored during investigation period. Anesthesia were induced with propofol given by TCI using Graseby 3500 infusion pump and remifentanil given by infusion pump.The target concentration of propofol was set at 2.5μg/ml. Anaesthesia was induced with remifentanil at infusion rate of 0.4μg.kg-1.min-1 after 3μg/kg Clonidine was intravenously premedicated. 5 min later propofol infusion was started.Vecurionium 0.1mg/kg was given intravenously to facilitate intubation. After intubation the remifentanil infusion was reduced to 0.2μg .kg-1.min-1. The propofol TCI infusion was adjusted by the feedback of BIS value.The total amount of propofol and remifentanil consumption and post anesthetic recovery time were recorded. Results The amount of propofol and remifentanil consumption in group I were significantly larger than those in group II(P<0.05). Either BIS or AEPex can be used for monitoring anesthesia depth for Remifentanil-propofol anesthesia . Conclusion After 3μg/kg Clonidine is intravenously premedicated, amount of Remifentanil and Propofol consumption are reduced, Stable haemodynamics during anesthesia is easily maintained and economical cost for patients is dropped.Simultaneous monitoring of BIS and AEPex can exactly respond to depth of anesthesia.
