| Objective To study the factors affecting the perioperative mortality in patients with esophageal and cardiac carcinoma. Methods from Jan.1985 to Jan.2005, 56 patients with perioperative death (death group) and 112 controls (control group) after esophagectomy and cardiectomy were studied. Age,sex,persistence time of symptom,degree of dysphagia,long-term heavy smoking,perioperative respiratory function,length of tumor,degree of differentiation,duration of operation,modality of anastomosis and site of anastomosis were compared by Logistic Regression analysis. Results The most frequent reason for perioperative death was respiratory complication and anastomotic leaking. The statistical results shows that the risk factors of perioperative death included severe dysphagia,bad preoperative pulmonary function,long-term heavy smoking and duration of operation more than 5 hours (P<0.05).Conclusion Improvement of nutritional status and preoperative pulmonary function,early withdrawal cigarettes and reducing operative time are the most important methods to minimize perioperative mortality in patients with esophageal and cardiac carcinoma. |
