Objective To investigate the clinical characteristics of polycystic ovary syndrome (PCOS) with insulin resistance.Methods Retrospective analysis is given out for PCOS in 48 patients. Insulin resistant group was consisted of 30 PCOS patients whose homeostasis model assessment of insulin resistance (HOMA-IR) was more than 3.8 or the ratio of area under curve of insulin concentration to that of glucose concentration(Auc G/Auc I) was less than 1.0. Non insulin resistant group was composed of 18 PCOS patients that are not fit both the above two standards. The body mass index, sex hormone levels, lipidic levels of the two groups were compared.Results (1)The body mass index of Insulin resistant group was higher than that of non insulin resistant group, there was significant difference between the two groups. (P<0.05)(2)The level of luteinizing hormone of Insulin resistant group was lower than that of non insulin resistant group, there was significant difference between the two groups. (P<0.05)(3)The level of testosteone of Insulin resistant group was higher than that of non insulin resistant group, there was significant difference between the two groups.(P<0.05)(4) The TG,ApoB of Insulin resistant group was higher than that of non insulin resistant group, there was significant difference between the two groups. ( P<0.05 ) (5)The high density lipoprotein-cholesterol(HDL),ApoA I of Insulin resistant group was lower than that of non insulin resistant group, there was significant difference between the two groups. (P<0.05)Conclusion (1) Obesity may be an important factor for insulin resistance;(2) There is remarkable difference of the sex hormone level between two groups of PCOS.: In insulin resistant group the elevated testosteone level is significant while the luteinizing hormone is the main change in non insulin resistant group.(3) It is easier for the patients with insulin resistance suffering from abnormal lipidic levels.
