Investigation Of Formation Mechanism Of Damp-heat Syndrome Of Southern Viral Upper Respiratory Tract Infection From Free Radical And Immunological Function

OjectiveViral upper respiratory tract infection which caused by virus can brings upper respiratory tract inflammation like rhinitis,pharyngitis and tonsillitis to some extent.Common cold,flu,AD viral infection,syncytial viral infection are some common infections in upper respiratory disease,of which the symptom can be mild or serious.At present to cure the disease,there is not a specific medicine.,however Chinese Medicine has a larger advantage than western Medicine in preventing and curing the disease.Upper respiratory tract infection belongs to exogenous disease category.It has similar symptom as some diseases in Chinese Medicine:anemogenous febrile disease,acute spring febrile disease,summer febrile disease,taiyang disease,shaoyang disease,yangming disease.In Chinese Medicine some experts hold that most causes of the disease are toxin,most differentiations of syndrome are dampness. Some southern areas like Guangdong and Fujian provinces,which lie in subtropical zone.The weather there is hot and rainy,people have the habit of drinking cool water and eating seafood,exterior and interior pathogenic factors make the spleen and stomach stranded by dampness.In conclusion, patients there get viral upper respiratory tract infection,most of which differentiations of syndrome are damp heat.Modern researches on damp heat indicate that external contraction of pathogenic dampness has direct connection with viral or bacterial infection.There are some index changes in damp heat syndrome:immunological function,free radical, interleukin.Chinese Medicine clearing away heat and expelling dampness can kill pathogenic microorganism directly,two-way regulate the immunological function,reduce free radical damage,help pathological tissue to recover. Patients who get viral upper respiratory tract infection have the same pathological foundation as those who are differentiated as damp heat syndrome. Damp heat pathogen has close connection with viral infection,which offer theoretical,clinical and experimental evidence on relativity between viral upper respiratory tract infection and damp heat syndrome.The condition of environment,immunologic system,metabolism and so many factors have contributed to the formation of damp heat syndrome synthetically. The disorder of digestive tract function,body lipoid and fluid metabolism has constituted as the physiological and pathological foundation of damp heat syndrome.The onste of viral upper respiratory tract infection is closely related to the state of immunologic system.The clinical manifestation and the pathognomonic feature of viral upper respiratory tract infection in southern area extremely resembles weifen and qifen syndrome in theory of seasonal febrile disease,most the differentiations of syndrome include dampness.Our research bases on the previous clinical experience,integrates dehygrosis with syndrome differentiation and mediation to cure viral upper respiratory tract infection.By way of prospective clinical research,we make an observation about patients who get viral upper respiratory tract infection and syndrome differentiated as damp heat.We mensurated the serum levels of T-lymphocytes CD3~+%,CD4~+%,CD8~+%,NK%,free radical SOD,MDA,NO,the viral antibody IgM and calculated CD4+/CD8+.Accordingly we may reveal the biochemical basis and mechanism of damp heat syndrome in viral upper respiratory tract infection.Wormwgod and Scutellaria Gallbladder Clearing Decoction can clear away heat and expel dampness,we use this decoction to cure the disease in order to prove the formation of substance of damp heat syndrome and guide the diagnosis and treatment of damp heat syndrome in viral upper respiratory tract infection.MethodWe selected 42 patients as case group,which diagnosed as viral upper respiratory tract infection from the medicine clinic and emergency clinic in First Affiliated Hospital of Guangzhou University of Chinese Medicine, GuangZhou Hospital of Integrated Traditional and Western Medicine,GuangDong Hospital of Traditional Chinese Medicine,and 20 normal cases as control group.Case group and control group were native people lived in southern areas in China.According to syndrome differentiation,case group was devided into two groups,which were damp-heat syndrome group(27cases)and warm-heat syndrome group(15cases).Indirect immunofluorescence was used to detect the blood serum for IgM antibody,T-lymphocyte subset was tested by flow cytometry, MDA,NO and SOD were determined by using bathyran chromatometry,nitrate reductase method,xanthine oxidase inhibition method.Compared the change of immune function and the level of free radical in damp-heat syndrome group,warm-heat syndrome group and control group.Wormwood and Scutellaria Gallbladder Clearing Decoction was used to cure Damp-heat syndrome group for three days,then used the same method to retest the T-lymphocyte subset and the level of free radical,Observed how the pricinple of clearing away heat and expelling dampness worked on damp-heat syndrome.ResultVirual antibody(IgM)positive rate was not significantly different in damp-heat syndrome group and warm-heat syndrome group(P＞0.05).Respectively compared Virual antibody(IgM)positive rate of four virus(influenza A, influenza B,adenovirus,Syncytial Virus),there was also no significant difference in damp-heat syndrome group and warm-heat syndrome group (P＞0.05).The level of SOD in damp-heat syndrome group was much lower and the level of MDA and NO in damp-heat syndrome group was much higher than that of the control group(P＜0.01),the level of NO was not significantly different in damp-heat syndrome group and warm-heat syndrome group(P＞0.05);The level of SOD in warm-heat syndrome group was much lower and the level of MDA and NO in warm -heat syndrome group was much higher than that of control group (P＜0.01),the level of NO was not significantly different in warm-heat syndrome group and control group(P＞0.05).About the change of T-lymphocyte subset,CD8~+%in damp-heat syndrome group was much lower than that of control group(P＜0.01),CD8~+%in warm-heat syndrome group was lower than that of control group(P＜0.05);Compared CD3~+%,CD4~+%,CD4~+/CD8~+,NK%,there was no significant difference in three groups(P＞0.05);after damp-heat syndrome group was cured by Wormwood and Scutellaria Gallbladder Clearing Decoction,the level of SOD was much higher and the level of MDA and NO was much lower than before(P＜0.01), CD3~+%,CD4~+%,CD8~+%,CD4~+/CD8~+ were on the trend of increasing,NK%was on the trend of decreasing,but there were no significant difference.(P＞0.05).Conclusion1.The pathological mechanism of damp-heat syndrome in upper respiratory tract infection has a certain link with peroxide radical damage,immune dysfunction, and this abnormal mechanism may induce the imbalance between yin and yang and the lingering and refractory pathological changes in damp-heat syndrome.2.Damp-heat syndrome in upper respiratory tract infection may have a strong connection with some respiratory virus infections,syndrome performance may related to the pathogenic features of the virus.3.The essence of damp-heat syndrome in upper respiratory tract infection relates to free radical damage,immune disorder,the pricinple of clearing away heat and expelling dampness can regulate the body' s immune function, reduce the free radical damage.
