Objective To study risk factors associated with AKI in patients after cardiac surgery with extracorporeal circulation.Methods A restrospective cases control study was carried out in patients who undwent cardiac surgery from 2003 to 2007 in our hospital,with 340 patients in AKI group and 4760 patients without AKI as control group.All variables were analyzed by univariete nanlysis,logistic regression or Mann-Whitney U test.Results AKI occurred in 340 patients,with an incidence of 6.7%.Univariate analysis revealed that age,preoperative serum creatinine,preoperative ejection fraction(EF),preoperativeβ2-microglobulin,preoperative blood abumin, preoperative blood uric acid,intraoperative cardiopulmonary bypass time,inteaoperative aortic cross-clamp time,postoperative Intubation time, postoperative hypotension,postoperative oxygen partial pressure(PO2),postoperative PH were significantly related to AKI following cardiac surgery with extracorporeal circulation,and logistic multivariate regression analysis showed that preoperative serum creatinine(p<0.000),preoperative ejection fraction(EF)(p<0.000), preoperativeβ2-microglobulin(p=0.002),preoperative blood uric acid(p=0.015), intraoperative cardiopulmonary bypass time(p<0.000),inteaoperative aortic cross-clamp time(p<0.000),postoperative hypotension(P=0.027),postoperative Intubation time(p<0.000)and postoperative oxygen partial pressure(PO2)(P<0.000) were independent risk factors of AKI.Conclusion preoperative serum creatinine,preoperative ejection fraction(EF), preoperativeβ2-microglobulin,preoperative blood uric acid,ntraoperative cardiopulmonary bypass time,inteaoperative aortic cross-clamp time,postoperative hypotension,postoperative Intubation time and postoperative oxygen partial pressure(PO2)were independent risk factors of AKI.
