Objective: To observe the effect of continuous propofol infusion on vital signs,glucose,serum lipids and acid-base balance . To investigate whether continuous propofol infusion is feasible or safe in clinical.Methods Experiment One: 60 patients scheduled for internal fixation or general surgery under general anesthesia were divided randomly into three groups: groupⅠ(control group),groupⅡand groupⅢ.In groupⅡ, patients were infused propofol in speed of 4~6mg.kg-1 .h-1. In groupⅢ, patients was infused propofol in speed of 8~10mg.kg-1 .h-1, Radial artery was cannulated for pressure monitoring and collecting blood samples. ECG,HR,BP,SPO2,PetCO2,BIS were continuously monitored throughout the operation.Arterial blood samples were taken for blood gas analysis and measuring Glu,TG,Lac and BE at five time points: T0 (before induction ), T1 (1 hour after propofol infusion), T2(2 hours after propofol infusion), T3(3 hours after propofol infusion ),T4(4 hours after propofol infusion).Experiment Two :30 SD rats were divided randomly into two groups: groupⅠ(control group) and groupⅡ. In groupⅡ, SD rats was infused propofol in speed of 50mg.kg-1 .h-1. Radial artery was cannulated for pressure monitoring and blood samples. ECG,HR,MAP,SPO2,PetCO2 were continuously monitored. Arterial blood samples were taken for blood gas analysis and measuring Glu,TG,Lac and BE at five time points: T0 (before induction ), T1 (1 hour after propofol infusion), T2(2 hours after propofol infusion), T3(3 hours after propofol infusion ),T4(4 hours after propofol infusion).Results Results of clinical experiments:There were no significantly changes among three groups on HR,SPO2,PetCO2 in the study. BP have differences between groupⅢand groupⅠ(P<0.05) . Propofol infusion in speed of 4-6mg.kg-1 .h-1 have small impact on surgery patients' TG There were differences between groupⅢand groupⅠ. TG was higher in groupⅢat T3 T4 than in groupⅡand groupⅠ(P<0.05) .In groupⅠ, Glu,Lac and BE after propofol infusion at T3 T4 was more higher than T0 (P<0.05) but it isn't at T1 T2 ; In groupⅡand groupⅢ, Glu,Lac,BE after propofol infusion at T0T1 T2 T3 T4 were not significantly differences.Results of animal experiments:In the SD rats experiments there were significantly differences between groupⅡand groupⅠon Glu,TG,Lac and BE . In groupⅠ,TG have not differences; Glu,Lac and BE after propofol infusion at T3 T4 were higher than T0 (P<0.05) but it isn't at T1 T2. In groupⅡ, Glu,TG,Lac and BE after propofol infusion at T1 T2 T3 T4 were more higher than T0 and have significant ifferences(P<0.01)Conclusions: This study suggests that in surgery continuous propofol infusion which does range in 4-10 mg.kg-1 .h-1 and less than 4 hours can effect the level of Glu,TG,Lac and BE but still all in normal.It can't increase the risk. With the increase of the passage of time and does ,the level of Glu,TG,Lac and BE have greater changed and some of them have exceed the normal levels.Popofol should be safely and effectively used to provide sedation in the intensive care unit and surgrey in order to avoid PRIS.
