Study On Building Up An Evaluation System Of Nursing Practice Environment

ObjectiveBased on the progress of nursing practice environment evaluation,the aim of this study is to make evalution tools that suitable to the current situation of China by the regulation of expert consultation.So as to give useful information for making strategic polices.Methods1.Delphi Method was used in this study.The interview were arranged for thirty-four representative experts from different working fields including nursing management,nursing practice,nursing eduation and nursig research.And they came from twelve geographically areas in China,such as Beijing,Shanghai,Chongqing,Guangzhou,Shenzhen,Shanxi and so on.2.A self-reported questionnaire was compiled based on the modle of nursing practice environmental, The sample for further evaluation on questionnaire recruited a total of 220 nurses from three general hospitals in Shanxi Province. Of these subjects,20 individuales were selected by random to research on reliability after two weeks.Results1.Evaluation on representative and dependability of experts'commentsIn our study, the percentage of experts with 20 years professional working experience is 46% and that with 30 years is 27%. They are higher level professional and representative in their field. The index dependability of their comments is 0.85 due to the expert's familiar with the questions that they were interviewed.The rate of qualified return sheet are 88% and 90% in two interview. This shows the result is statistically valuable, while it is suggests the experts support our study .In the return sheets,the experts gave more than 100 constructive comments, again expressed their intense about this investigation. The cooperation index of the expert are 0.234 and 0.253(p<0.001).It implys that the result is valuable.2.Selection and definition on indexLikert 5-scale method was used to describe the expert's opinion of the importance of the item. The mean of 3.50 and the coefficient of 0.20 were viewd as cut-off point and the items with more than 3.50 and less than 0.20 were selected to further research . Moreover, the weight of items was identified with using the Delphi Method. The weight of the first grade includingteam work,reward work, management, professional and personaldevelopment,nurses involved in the Hospital services scheduleing and work safety, were 0.1898,0.1688,0.1679,0.1348,0.1236,0.1101,0.1051 respectively.,while the weight of the second grade were identified throught the same methods.3.EvaluationIn our study, the value of test-retest reliability , Cronbach alpha and split-half reliability were0.877,0.915 and 0.941 respectively. The factor analysis result showed that seven factors were selected. The cumulative extraction sum of squared loadings was 52.94%and it reached to 55.92% after amended.These results suggested that configuration of questionnaire was accordant to the result of factor analysis in the main. The questionnaire had a better reliability and construct validity.Conclusions1.The evaluation system from our study definite 42 items, including 7 items of the first grade 35 items of the second.2. Both rationality and feasibility of the index system were verificated in the study,and the assessment model had better reliability and validity.
