[OBJECTIVE]To investigate the safety and efficacy of prostate volume>80ml surgerical treatment by TUVRP/TUPKP for benign prostatic hyperplasia (BPH ).[METHODS]A retrospective review was conducted of 189 BPH cases.Among the total,the prostate volume>80ml cases are 119,treated by SPP (n=56) and TUVRP or TUPKP(n=63).The left 70 cases of prostate volume<80ml were become the control group,treated by TUVRP/TUPKP.All patients were compare with operation time,blood loss, volume of blood transfusion, indwelling catheterization, hospital stay ,curative effect and operative complications.To estimate the advantages and disadvantages of each group.[RESULTS](1)IPSS, QOL, Qmax and RUV were significantly improved after either of the procedures, no significant difference in the improvement of effects and complications had been noted with the different procedure (P >0.05). (2) In compare with the two endovascular surgery of prostate volume<80ml and prostate volume>80ml ,only the mean operative time and blood loss of the former less than the latter(P<0.05) .(3)The blood loss and volume of blood transfusion of SPP more than endovascular surgery, mean operative time,indwelling catheterization and length of stay was longer for SPP than TUVRP/TUPKP (P<0.05).[CONCLUSIONS](1)The open surgery and endovascular surgery were all effective for the treatment of BPH.SPP has more operation injury and longer duration of recovery. (2) Endovascular surgery to BPH of prostate volume>80ml is feasible in the case of the operator has perfect operation skill and the hospital has advanced equipments.The curative effect of intraoperation treatment is as same as the open surgery ,but the endovascular surgery have the excellences of small wound, short hospital stay and rapid renew.
