Study On The Feasibility Of The Self Rating Depression Scale(SDS) As A Conventional Screened Implement For Depressive Disorder Of Surgical Inpatients

[OBJECTIVE]To investigate the incidencerate of depressive disorder in patients of general hospital,analyze the related factors;to explore the feasibility of the self rating depression scale(SDS)as a conventional screened implement for depressive disorder of the patients in general hospital.[METHODS]A total of 863 patients satisfying conditions were sampled from a certain provincial general hospital and they were evaluated with a basic information questionary and the self rating depression scale(SDS),711 patients of them finishing the investigation.Then,711 patients were divided into positive groups and negative groups,based on the result screened by SDS.Psychiatrists analyzed possible influencing factors of depressive disorder about the groups. The subjects screened by SDS positive and 20%of those screened negative were diagnosed with Structured Clinical Interview for DSM—ⅣAxisⅠDisorders(SCID) to generate a gold standard diagnosis;the efficiency to diagnose depressive disorder of patients in general hospital between SDS and SCID is compared. Datebase was analyzed by general describing,T-test,Chi-square,Binary Logistic Regression and analysis of variance using SPSS13.0.[RESULTS]1.Results screened by SDS:A total of 711 patients finished the investigation.The average standard scores of depression were 45.96±12.42. The number of the patients with depressive symptom was 251,and the total rate of depression was 35.3%.During the subjects,the rate of low depression,moderate depression and high depression is 18.85%,11.11%and 5.34%.During the 251 patients with depressive disorder,only 7 patients were identified by physician,in which 2 patients diagnosed as depression,3 patients diagnosed as depressive neuroses,1 patient diagnosed as neuroses and 1 patient diagnosed as anxiety.2.The related factors of depressive symptom in general hospital:The unvaried analysis showed that depression were associated with gender,age,education level,economic condition of family,hospitalization duration,previous history and recrudescence.Logistic regression analysis showed that gender,education level,economic condition of family,hospitalization duration,previous history and recrudescence were the main factors.3.The diagnostic result of SCID:343 patients screened by SDS positive and 20%of those screened negative were diagnosed with SCID;the number of the patients with depressive disorder was 243 and the rate of the depressive disorder was 39.24%.During the patients diagnosed by SCID,the number of the positive subjects screened by SDS,which was diagnosed as depressive disorder,was 234;the number of the negative subjects screened by SDS,which was diagnosed as depressive disorder,was 9.The consistency between SDS and SCID to screen depressive disorder was high and the Kappa is 0.812.The agreement rate,positive predictive value and negative predictive value is 92%,0.93 and 0.90.[CONCLUSION]1.The incidence rate of depressive disorder in the general hospital is high.The standard score of SDS in the patients were significantly higher than those in the Chinese norm.The rate of recognition and treatment for depressive disorders of patients in general hospital is low.2.The main influcing factors of depressive symptom in patients of general hospital were gender,education level,economic condition of family,hospitalization duration,previous history and recrudescence.3.The consistency between SDS and SCID to screen depressive disorder was high.SDS could be employed as a diagnostic screened implement for depressive disorder of patients in general hospital.
