| Part one: In order to get the clinical information and patch test results of cosmetics dermatosis, we summarized 327 cases of cosmetic dermatosis in Beijing 2006. Results: (1) 98.47% of the patients are female,most of the patients aged from 21 to 40 years old. (2) The suspected cosmetics included 47 brands with 618 imported products,43 brands with 293 domestic and joint enterprise products,and 14 illegal products. (3) Clinically 318 cases (97.24%) of the patients were diagnosed as cosmetic contact dermatitis,7 cases (2.14%) of acne, 2 cases (0.61%) of skin discoloration. (4) Totally, 312 patients were patch tested with the products they used, 15 patients were not patch tested. 192 (58.72%) patients showed positive patch test results, 120 (36.7%) patients were negative. We performed patch test with 887 products of different brand, of which 267 (28.86%) products were positive, 620 (67.02%) products were negative.Part two: In research part one, we found that some patients with cosmetic contact facial dermatitis had definite history of contacting cosmetics, and clinical situation were very typical,but the results of patch test were negative. Patients often apply products on their faces, but when we do patch test, we choose the back. There are many differences between the skin of face and back, such as the thickness of skin. Many factors can affect the result of patch test, such as reading time, dose and volume of allergen, concentration of allergen and patch test unit, but there is no study about whether the site of patch test can affect the result of PT. We performed patch tests with the same products that patients used on their face, forearm and back. Totally, 112 patients finished patch tests with 318 products which they had used. Results: (1) The highest rate of positive reaction appeared on the face. There were 62 patients (55.36%) and 84 products (26.42%) showed positive reaction on face, 39 patients (34.82%) and 59 products (18.55%) showed positive reaction on forearm, 38 patients (33.93%) and 52 products (16.35%) showed positive reaction on back. (2)Statistically, there was no difference between the results of patch tests applied on back and forearm. There are significant differences of PT results on forearm and back from that on face.Part three: 47 patients with suspected cosmetic contact dermatitis were further studied. Different PTs were carried out with suspected cosmetics, European standard allergens and Cosmetic-related allergens. Results: (1) 16 patients (34.04%) claimed they once had different degree of alleregic reations , 9 patients (19.15%) claimed they once had different kind of dermatosis,such as eczema, urticaria and so on.5 patients (10.64%) claimed their skin were sensitive,they were allergic to one or more than one cosmetics before. (2) The PT results of 46 products (28.75%) of 27 patients (57.45%) were positive. (3) When reading on the third day, the results of 39 products (84.78%) of 24 patients were still positive. (4) The results of 6 products (13.04%) of 4 patients on second day were positive, but on third day were negative or doubtful. (5) The result of 1 product on second and third day was negative, but on forth day was positive. (6) 41 patients (87.23%) showed positive reaction to one or more Cosmetic-related allergens, the highest rate of positive reaction is Sorbitan sesquioleate (53.19%). (7) 43 patients (91.49%) showed positive reaction to one or more European standard allergens, the highest rate of positive reaction is Cobalt chloride (68.09%). (8) Among 27 patients positive to the products, 23 ones showed positive reactions to one or more Cosmetic-related allergens. The allergens are preservative, emulsifier, perfume, surface active agent, etc. Among 11 patients with doubtful results to the products, 10 ones showed positive reactions to one or more Cosmetic-related allergens. Among 9 patients with negative results to the products, 7 ones showed positive reaction to one or more Cosmetic-related allergens. (9) Among 27 patients positive to the products, 25 showed positive reaction to one or more European standard allergens. Some allergens, to which 21 patients showed positive reaction, belong to Cosmetic-related allergens. 11 patients with doubtful results to the products, 9 showed positive reactions to one or more European standard allergens. Some allergens, to which 8 patients showed positive reaction, belong to Cosmetic-related allergens. 9 patients with negative results to the products showed positive reactions to one or more European standard allergens. Some allergens, to which 5 patients showed positive reaction, belong to Cosmetic-related allergens.Conclusion: (1) the young females aged from 21 to 40 years old with middle or high education are the most susceptible people to cosmetic dermatosis. (2) In all suspected products, sunscreen and moisturizer were the most highly suspected. (3) Cosmetic contact dermatitis is the most common cosmetic dermatosis. (4) The sensitivity of different site of skin has a direct impact on the result of PT, the facial skin is more sensitive than forearm and back. (5) For some cosmetic contact facial dermatitis, PT on face may be recommended. (6) It is of significant importance to perform PT with the patients'products. (7) To diagnose cosmetic contact dermatitis, we should combine the PT results of products with Cosmetic-related allergens. (8) To diagnose cosmetic contact dermatitis, we should also perform PT with European standard allergens. |
