Background and objective: Trauma is a major disease harming human health and lives and the leading cause of death for people at less than 40 years old. Therefore, it is of necessity to understand law and characteristics of trauma happen, care and outcome so as to prevent and cure it effectively. In American, the trauma registry has been established in 1970s in order to investigate trauma, based on which MTOS has been developed for studying outcome of trauma. Widespread application of computer and increasing improvement of database has widened the serviceable range of trauma database that becomes a platform for extensive epidemiologic study rather than only for trauma outcome. At present, there is no trauma database that contains overall trauma data and can represent the current situation of trauma care and the research direction of trauma in China, which results in waste of great quantity of vital trauma data. The objective of this study is to investigate the content and structure of database that can represent the state of trauma research and care in China and cater for effective registration of trauma data so as to establish a new trauma database facilitating trauma research and care in China.Methods: 1. Scientific and rational content and structure of trauma database was investigated and formulated by analyzing the current and developmental state of trauma research and care in China and consulting the experiences and theory of constructing trauma database in China and foreign countries. 2. The trauma database was developed based on the MS Access, with JavaScript to develop the application. 3. Through registering and analyzing trauma data from Daping Hospital, Chongqing, in 2005, the performance of the database in registration, administration and analysis was checked to preliminarily discuss the condition of trauma care in Daping Hospital.Results: 1. We developed a new database with the content and structure of trauma database that could relatively represent whole trauma care process. The content of the database involved 10 parts, 21 tables and 234 fields. Ten parts included patient data, pre-hospital condition, injury severity, transfer hospital, emergency admission, hospital care, diagnose and scoring, complication and comorbidity, care quality and discharge. 2. Based on the content and structure of trauma database, a registry and administration system of trauma database was developed, with functions of security verification, data administration, input and output of data and statistical analysis and with data registration mode of Browse/Server. 3. By registering and analyzing trauma data of 846 patients, it was proved that the trauma database could collect trauma data relatively comprehensively and scientifically, had capacity of administrating and analyzing data. At the same time, we confronted some difficulties and problems in collecting overall trauma data. 4. Analysis of trauma data preliminarily showed general law and characteristics of trauma patients in Daping Hospital, which could be helpful for hospital arranging trauma care.Conclusions: It is proved, by developing trauma database and registering and analysing trauma data, that it is feasible to establish a trauma database suitable to the current state and requirements of trauma care in China. Of trauma database, content is scientidfic and copious, function is practical, structure is rational and relation between tables is clear. The interface of database, in which registering data is simple and prompt, is friendly and convenient for us to collect, inquire and analyse trauma data, and refers to clinic well. The trauma database can standard the input and elementally satisfy the demands of trauma research in China, such as helping understand epidemiological characteristics of trauma, studying and analysing factors affecting trauma outcomes, improving level of trauma care, rationally allocating health resources and providing reference basis for decision-makers framing relating polices.
