Objective: To study the mid-term follow-up results of cemented total hip replacement (THR) and to analysis the incidence of osteolysis in different parts.Compared to the results of medium-term follow-up of uncementd total hip arthroplasty ,to analysis the differences of different diseases with cementd and uncementd total hip arthroplasty.Methods: 47 cases were involved the follow-up (age from 47 to 69 years, average 62.3y, follow-up 5-7years) in our institution from 2000 to 2003. The group divided into fresh femoral neck fracture (FFNF), old femoral neck fracture (OFNF), osteoarthritis (OA) and femoral head necrosis (FHN) according to the diagnosis. Therapeutic evaluation was measured by using Harris score.Roentgenographic feature of osteolysis around the components was observed. The incidence of liner osteolysis and expansile osteolysis around the components in different regions was statistically determined.Compared to the results of medium-term follow-up of uncementd total hip arthroplasty, to analysis the differences of different diseases with cementd and uncementd total hip arthroplasty.Results: Average Harris score recovered from 26.51 to 80.06 in FFNF group, 37.73 to 89.50 in OFNF group, 40.57 to 81.18 in FHN group, 35.0 to 78.83 in OA group respectively. The score was higher in OFNF group (p<0.05).The incidence of osteolysis is 63.9%,and 57.4% in the acetabular, 42.5% in the femoral. By osteolysis types, occurred in 28 cases of varying degrees of osteolysis contrast, accounting for 59.5%, of which 23 cases of acetabular side, 18 cases of proximal femoral; 21 cases of expansile osteolysis occurred, accounting for 44.7%, of which lateral acetabular 18 cases, 15 cases of proximal femoral. Different types of disease, fresh group of 12 cases of femoral neck fracture osteolysis occurred, accounting for 66.7% of old femoral neck fracture group osteolysis occurred in seven cases, accounting for 58.3%, 8 cases of femoral head necrosis group osteolysis occurred, accounting for 72.7 %, 3 cases of osteoarthritis osteolysis occurred, accounting for 50.0%.There was no significant difference among the incidence of liner osteolysis of each zone at acetabulum (p>0.05). The incidence of expansile osteolysis was the highest in zone 3 (p<0.05). The incidence of liner osteolysis was higher in zone 2,6,5,7. (p<0.05) at proximal femur. The incidence of liner osteolysis was higher in zone 1,7,6. (p<0.05). There was no significant difference among the incidence of osteolysis of each diease (p>0.05). Conclusion:1,Cemented THR is effective on treating the hip disease.,with a good mid-term follow-up results.2,The incidence of osteolysis is 63.9%,and 57.4% in the acetabular, 42.5% in the femoral. There was no significant difference among the incidence of osteolysis of each diease (p>0.05).
