Effect Of Helicobacter Pylori Infection On Survivin And Elf-1 Protein Expression In Gastric Carcinoma And Its Significance

Objective To study the effect of Helicobaeter pylori infection on Survivin and Elf-1 protein expression in gastric carcinoma and its significance.Methods The Survivin and Elf-1 protein expression in gastric cancer tissues and those adjacent tissues and distal mucosa were investigated by immunohistochemistry in 70 cases diagnosed as gastric carcinoma,and the relationship between Survivin and Elf-1 expression in the malignant tissues and patient's pathological characters were evaluated.The Helicobacter pylori infection was detected by urease test and Warthin-Starry staining,and their relationship with Survivin and Elf-1 protein expression were studied.Results 1.Survivin was mainly expressed in cytoplasm and nucleus.The positive rate of Survivin expression in the gastric cancer tissues was 74.3%and that in adjacent tissues was 41.4%,but Survivin didn't express in the distal mucosa.The expression value of Survivin in gastric cancer tissues was higher than that in the adjacent tissues(X~2=33.299,P = 0.000) and was more intensely expressed(Z=5.329,P = 0.000);Elf-1 was mainly expressed in nucleus and oecasionally in cytoplasm.The positive rates of Elf-1 expression in the gastric cancer tissues,those adjacent tissues and distal mucosa were 88.6%,55.7%and 5%,respectively, and significant difference existed among the three groups(X~2=78.553,P=0.000).The positive rates of Elf-1 expression in the gastric cancer tissues was significantly higher than that in those adjacent tissues(X~2=22.960,P = 0.007 )and that in the distal mueosa(X~2=79.226,P = 0.000).The positive rate of Elf-1 expression in the adjacent tissues was significantly higher than that in the distal mueosa(x=28.001,P=0.000).There were differences among the expression intensity of the three groups(He=70.1072, P=0.000).The expression intensity of Elf-1 in the gastric cancer tissues was higher than that in the adjacent tissues(q=7.2170,P=0.0001) and that in the distal mueosa(q=17.9304,P=0.0000),The expression intensity in the adjacent tissues ass significantly higher than that in the distal mueosa(q=17.3773,P=-0.0001).2.Both Survivin and Elf-1 protein expression were related to histopathologieal gradings(u=-4.5163, P＜0.01;u=-4.8097,P＜0.01),depth of infiltration(HC=14.7093,degree of freedom=2, P＜0.01;Hc=19.6229,degrees of freedom=2,P＜0.01),clinical staging (u=-1.9631,P＜0.01;u=-4.5367,P＜0.01) and lymph node metastasis(u=-3.4229, P＜0.01;u=-4.4384,P＜0.01),but not related to sexes of patients(u=-1.2667,P＞0.05; u=-0.0406,P＞0.05),Borrmann staging(Hc=2.6090,degree of freedom=3,P＞0.05; Hc=8.0919,degree of freedom =3,P＞0.05).The Survivin and Elf-1 expression were higher in lower histopathological gradings,deeper infiltration,later clinical pathological stages, and more lymph node metastasis of gastric cancer.3.In gastric carcinoma tissue with positive Hp,both the positive rate of Survivin and Elf-1 protein expression (x2=9.441,P=0.024;x2=4.6082,P=0.0318) and their expression intensity(Z=2.460,P = 0.014;u=-3.4887,P＜0.01) were higher than those in the gastric cancer tissues with negative Hp,the result being statistically significant.4.Remarkable correlation was found between the expression of Survivin and Elf-1 in gastric carcinoma tissues(r_s= 0.4569,P=0.0000),and also in gastric carcinoma tissues with positive Hp(r_s=0.313,P=-0.032).Conclusions:1.The positive rate of Survivin and Elf-1 protein expression become higher and their expression more intense from gastric cancer tissues,those adjacent tissues to distal mucosa,suggesting that the Survivin and Elf-1 protein expression is correlated with the tumorigenesis of gastric carcinoma.2. The expression of Survivin and Elf-1 in the gastric cancer tissues is related to histopathological gradings,depth of infiltration,clinical staging and lymph node metastasis,which reveals that the Survivin and Elf-1 protein expression is associated with the progression of gastric carcinoma.3.The infection of Helicobacter pylori(HP) can increase the expression of Survivin and Elf-1.4.The Survivin and Elf-1 protein expression may coordinate in the tumorigenesis and progression of gastric carcinoma.It is significant to detect the expression value of Survivin and Elf-1 for early diagnosis and prognosis assess of gastric carcinoma.
