Objective Through the investigation of Helicobacter pylori infection status and related risk factors in Yan’an residents,the characteristics of Helicobacter pylori infection and the expression of gastric function in patients with chronic atrophic gastritis in Yan’an were investigated,and the diagnosis and treatment of chronic atrophic gastritis and Helicobacter pylori infection were made.MethodsPatients who were admitted to Yan’an University Affiliated Hospital,Yan’an People’s Hospital,Yan’an Traditional Chinese Medicine Hospital and Baota District People’s Hospital in 2018 were diagnosed as chronic atrophic gastritis by gastroscopy,and tested by rapid urease test or C14 breath test.A questionnaire survey was conducted for patients with Helicobacter pylori positive.To analyze the relationship between various risk factors of Helicobacter pylori infection and chronic atrophic gastritis.Three patients with positive gastric function were enrolled in the outpatient department of our hospital who completed the 14C-expiratory test and were confirmed by gastroscopy to be chronic atrophic gastritis or non-atrophic gastritis.After calculating the area of ROC curve,the value of three functions of gastric function combined with Helicobacter pylori DPM was used to diagnose the value of chronic atrophic gastritis.A total of 54 patients from the above 337 patients were enrolled in the Helicobacter pylori virulence genotyping test to calculate the area of the ROC curve,and to analyze the value of the combination of gastric function and Helicobacter pylori typing for predicting chronic atrophic gastritis.Results1.In this survey,a total of 13281 residents with chronic atrophic gastritis diagnosedby endoscopy in Yan’an City were collected in 2018,aged 18-86 years,mean age(52.24±10.12).796 cases of general information and information,gastroscopy,Helicobacter pylori detection,etc.were selected to analyze the Helicobacter pylori infection.Among them,412 cases were positive for Helicobacter pylori,and the total infection rate was(51.76%).Univariate analysis of 412 subjects showed that the cultural level,family economic income,eating habits(picks,leftovers),family history,and shared tableware at home were statistically significant(P<0.05).2.The indexes of PGI,PGII,PGR and HP(DPM)were significantly different between the two groups in atrophic gastritis and non-atrophic gastritis,and the difference was statistically significant(P<0.05).3.Helicobacter pylori type I and type II were not significantly different between atrophic gastritis and non-atrophic gastritis,and the difference was not statistically significant(P>0.05).Conclusion1.According to the epidemiological survey data of Yan’an City,Helicobacter pylori infection in Yan’an City is related to occupational category,cultural level,family economic income,eating habits(picks,leftovers),family history,and shared tableware at home.Risk factors for Helicobacter pylori infection in residents of Yan’an City.2.Helicobacter pylori infection is associated with chronic atrophic gastritis.The decrease of PGI,PGR and the increase of G-17 are associated with atrophic changes of gastric mucosa.Combined HP(DPM value)and gastric function can effectively predict changes in gastric mucosal atrophy.3.No clear relationship between Helicobacter pylori typing and chronic atrophic gastritis was found. |
