The Relationship Between The Position Of Patella and Chondromalacia PatellaPurpose:To summarize the manifestation of chondromalacia patella on MRI,carry out the epidemiologic study of chondromalacia patella,analysis the relationship between the position of patella and chondromalacia patella,expound its mechanism and approach its clinical application.Materials and Methods:1052 examiners do MRI of knee joint,among the total there are 299 patients having chondromalacia patella.Calculate prevalence by groups,and then use Insall-Salvati method to measure all examiners' numeruses of L,which means the length of ligament patellae from the anus perineum of patella to tibial tubercle(TT) and P which means the longitudinal path length of patella.Calculate the numeruses of L/P.Then,dividing them into groups by sexuality and age,compare their means,at last use t test andχ~2 test to make judgment whether the relationship between the position of patella and chonromalacia has the statistical significance by use both the traditional normal range and the normal range in our research.Results:The whole masculine rate of chonclromalacia patella is 28.4%,female is 36.4%and male is 19.8%.The masculine rate of female is higher than male.(P＜0.01) Through the increasing of age,the masculine rate becomes higher.The normal range of Insall-Salvati index number in our research is 1.15+0.15.Female is higher than male.The normal range of Insall-Salvati index number in every age group is also different.In every group,the mean of Insall-Salvati index number of the group of chondromalacia patella is higher than it of the normal group.(P＜0.01) Except it in the group from 0 to 19 years old,the researches in the other group dividing by sexuality and age all show that there has the conspicuous relationship between the position of patella and chondromalacia patella.(P＜0.01) Among it,there has the conspicuous relationship between high patella and chondromalacia patella(P＜0.01),but there hasn't the conspicuous relationship between low patella and chondromalacia patella (P＞0.05).Conclusion:Chondronmalacia patella has high masculine rate.Moreover,female is higher than male.Through the increasing of age,the masculine rate becomes higher. In our research,we produce that the normal range of Insall-Salvati index number is different from the traditional range.Female is higher than male.the normal range of Insall-Salvati index number in every age group is also different.High patella is relevant to chondromalacia patella.High patella makes the pressure of patellofemoral joint increasing,the change of the stress can make injury because of the increasing of the pressure of cartilage.Chondromalacia patella may develop for long.In our research,the etiopathogy of chondromalacia patella is introduced,leading to diagnosis and judgment forproper time of theropy which aiming directly at the etiological factor. MRI is the best imaging method to diagnose pathological changes of patella by now. The manifestation of chondromalacia patella on MRI has some invariably mark. Using MRI to measure Insall-Salvati index number is a reliable indicator for diagnosis of chondromalacia patella.
