1.ObjectiveTo investigate the association between IMA levels and NT-proBNP or left ventricular ejection fraction in patients with unstable angina and who developed heart failure during their CCU stay.2.MethodsWe studied 54 patients with unstable angina who underwent a successful CAG with 24h of the onset of chest pain.None of patients had abnormal renal function, stoke,transient ischemic attack or peripheral vascular disease,AMI, hypoalbuminaemia,and so on.The presence of heart failure was assessed by 2 cardiologist on CCU admission.Group HF(n=32) comprised patients in NYHA classâ… ,and group NHF(n=22) NYHA classesï¼žâ… .Blood samples were drawn from a cubital median vein 3 h of the onset of pain for IMA,NT-proBNP measurement into BD vacutainer tubes(BD vacutainer SSTâ…¡,BD,USA),without any anticoagulant.3.ResultsThere are significant difference in IMA levels(Group HF:541.67±120.98, Group NHF:413.05±156.52,P=0.001),NT-proBNP levels(lg NT-proBNP Group HF:3.43±0.80,Group NHF:2.06±0.46,P=0.000),LVEF(Group HF:0.50±0.14, Group NHF:0.69±0.06,P=0.000) between patients with heart failure and those without heart failure.There was significant difference among NYHA function class grades.There is linear relevant between IMA and NT-proBNP or LVEF.The cut off value of IMA is 373.72mg/ml,the sensitivity of IMA for diagnosing Heart Failure is 96.9%,the specificity is 50%,the positive predictive value is 73.8%,the negative predictive value is 91.7%.4.Conclusion1).In patients with unstable angina,serum IMA levels are significantly related to NT-proBNP or left ventricular ejection fraction and represent an early markers of left ventricular dysfunction.2).The higher the NYHA function class goes,the higher IMA levels is.There is correlation between IMA and NYHA function class.
