Backgroud Osteoarthritis is one generally disabling disease characterized by gradually articular cartilage destruction.This disease is the second leading cause of loss of labor force in US,which amount of hospital population is far higher than that of rheumatism arthritis and populations above 50 years old involve about 21 millions. In china,osteoarthritis mainly concentrate in elder population,and the prevalence of osteoarthritis in general population is 4%,which>50%in population above 65 years old and>80%in that above 75 years old.The prevalence of OA is 9.6%and 49% respectively in Chinese country population and urban retired population.The early diagnosis of OA is one medical tough problem,and currently diagnosis options are mainly based on imaging detection and occurrence of clinical symptom.However, these ways is unavailable for early detection,and always lead to loss of good opportunities of early treatments and poor post-treatment effect.Therefore,some measurable,sensitive and specific markers are required for early diagnose of disease in clinic.The detection of BM of Synovial fluid,serum and urine is useful for early diagnose of disease,monitoring of disease progress and drug reaction.The BM of Synovial fluid could directly reflect the intra-articular pathological change.Objectives 1 The Research on the Relationship between the HELIX-Ⅱ(typeⅡcollagen helical peptide,HELIX-Ⅱ) levels in the synovial fluid of the knee and the degree of cartilage injury and synovitis.2 To explore the relationship between the HELIX-Ⅱlevels in the synovial fluid of the knee and the degree of osteoarthritis.Methods 42 patients with knee osteoarthritis(OA group) and 41patients with other knee diseases(non-OA group) were operated by arthroscopy and TKR were studied.The HELIX-Ⅱlevels in the synovial fluid were measured by enzyme linked immunoassay.The degree of cartilage injury was assessed with Outerbridge's score system,the degree of synovitis with Ayral's score system under arthroscopy and the classification of radiographic progression were made by Kellgren-Lawrance.The data were analyzed by t-test,analysis of variance and Spearman correlation.Results The results show that the HELIX-Ⅱlevels in the synovial fluid of the knees are correlated positively with accumulative Outerbridge's score(r=0.81, P<0.05),with Ayral's score(r=0.85,P<0.05),with Kellgren-Lawrance's classification (r=0.8,P<0.05),and with maximum Outerbridge's score(r=0.79,P<0.05).The HELIX-Ⅱlevels are higher in those samples with accumulative Outerbridge's score great than or equal to 10 than others with the score less than 10(P<0.05).It is higher in those samples with maximum Outerbridge's score great than or equal to 2 than others with the score less than 2(P<0.05).It is higher in patients with Ayral's score great than or equal to 60 than in those with the Score less than 60(P<0.05).In Kellgren-Lawrance's classification,the significant differences of HELIX-Ⅱlevels were found among the grades groups,besides that between grades land 2 and grade 3 and 4.The level in those samples with maximum Outerbridge's score less than 2 and K-L's classification 0 grade is about 0.5 nmol/l.The HELIX-Ⅱlevels in the synovial fluid of the knees of OA group were higher than that of non-OA group(P<0.05).The HELIX-Ⅱlevels in the synovial fluid of OA are correlated positively with cartilage injury scores(r=0.41,P<0.05),with synovitis scores(r=0.36,P<0.05).Conclusions The HELIX-Ⅱlevels in the synovial fluid of the knee can be regarded as a biomarker to reflect the degree of accumulative cartilage injury, synovitis and to be associated with increased risk of radiographic progression in OA, especially accumulative cartilage injury.The accumulative cartilage injury is more severe when the HELIX-Ⅱlevels in the synovial fluid of the knee are increasing. Maybe,we can use it to predict the development of OA and to appraise the curative effect of treatment.
