Background:Osteoarthritis is also called degenerative arthritis or joint disease,the incidence increased significantly with growing old.Osteoarthritis is the most common cause that leads to the elderly chronic joint pain and dysfunction.In recent years,although there are a great deal of research about knee osteoarthritis causes,the osteoarthritis pathogenesis is not clear for the moment.With the deterioration of the disease,the treatment often need surgeries,such as TKA and THA etal.At present,the cost of joint prosthesis and related treatment is expensive,which brings great spiritual and economic burden to family and society.Joint replacement is also facing many problems,especially for young patients,due to the limitation of prosthesis life and compications of postoperation.Therefore,it is of great significance to study the pathogenesis of osteoarthritis in order to control the progression of osteoarthritis.Inflammatory cell exudation is a marker of inflammation,and it is a useful means to control the inflammatory cell aggregation to alleviate the inflammatory reaction.It is important to study the metabolic signal pathway of cartilage.Objective:To study the roles of Chemerin,chemR23 and PPAR gamma 2in the osteoarthritis pathogenesis,the expression of mRNA and protein of Chemerin,chemR23 and PPAR gamma 2 in cartilage were detected,and the concentration of chemerin in serum and synovial fluid(SF)were detected too.Method:This study collected the articular cartilage,SF and serum as study samples.Patients’ knee cartilage were seen as the experimental group,who underwent the knee arthroplasty because of knee osteoarthritis.The control group include femoral head cartilage and normal cartilage of amputated lower limbs.The serum and synovial fluid was collected when Sodium Hyaluronate Injection was injected.The expression of Chemerin,chemR23 and PPAR gamma 2 mRNA in cartilage was detected by RT-PCR,and the expression of Chemerin and PPAR gamma 2 protein in cartilage were detected by Western-blot.The concentration of chemerin in serum and synovial fluid in both healthy and KOA was detected by Enzyme-linked immunosorbent assay.Result : There was no significant difference in age,sex and body mass index between the experimental group and the control group.(1)Chemerin,chemR23 and PPAR gamma 2 were expressed in articular cartilage。(2)The results of RT-PCR by calculating the 2-Delta CT showed that Chemerin and PPAR gamma 2 of the OA group is higher than that of the control group,the difference was statistically significant(P < 0.05),but there is no significant difference in ChemR23 expression between two groups(P > 0.05).(3)The expression of Chemerin protein in OA cartilage was higher than in the control group,the expression of PPAR gamma 2 protein in OA cartilage was higher than that in control group,the difference was statistically significant(P < 0.05).(4)Spearman correlation analysis showed that there was a positive relationship between Chemerin and PPAR gamma 2(r=0.542,P<0.001).(5)The chemerinconcentration in SF of K-L 4 KOA patients is significantly higher than that of K-L2 and K-L3.And there exists a positive liner relationship between Chemerin and Kellgren-Lawrence grading of KOA(r=0.367,P<0.001).Conclusion:(1)The concentration of chemerin in SF increases with the degree of knee osteoarthritis by K-L grading,And there exists a positive liner relationship between Chemerin and Kellgren-Lawrence grading of KOA.(2)The Chemerin and its receptor chemR23 are both expressed in human cartilage.(3)The expression levels of chemerin and PPAR gamma 2 in osteoarthritis cartilage were higher than the control,and there exists a positive liner relationship between Chemerin and PPAR gamma 2.Chemerin,as an inflammatory cytokine,may plays an important role in the occurrence and development of KOA.The chemerin concentration in SF may be a new index to judge the degree of osteoarthritis. |
