Objective : To explore the effectiveness and significance of neuronavigation in transsphenoidal approach microneurosurgical treatment of pituitary adenoma.Methods :39 patients with pituitary adenoma underwent transsphenoidal surgery, including sublabial transseptal approach in 5,transnasal submucosal transseptal approach in 11,and transnasal septal displacement approach in 23,in which StealthStation neuronavigator was use to localize the anatomic midline, anterior-wall of sphenoidal sinus, floor of sella, cavernous sinus, internal carotid artery, clivus,and to estimate the extend of lesion-resection.Results: The mean fiducial error were in 2.14mm, range from 1.06 to 3.22mm. The operative time was 66~92 min (mean, 79 min). A total tumor resection was achieved in 30 patients and a subtotal removal in 9. No severe complication in all the cases, transient polyuria happened in 9 cases.Conclusion:The application of neuronavigation in the transsphenoidal surgery for resection of pituitary adenoma can dramatically improve the security of transsphenoidal approaches , decrease surgical complications and expand the range of trans-sphenoidal approach applying.
