Objective: Psoriasis is a common,chornic,recurrent,inflammatory disease of skin, which characterized by round circumscribed erythematous, dry scaling plaque of various size covered by grayish white or silvery white,imbricated and lamellar scales. The lesion have a predilection for scalp,extensor surface of the limbs (especially the shins),elbows,knees and so on. Conventional treatment often to stand up effect slowly and easy to recur. The Pathogenesis of psoriasis is still not yield certain conclusion, but immunology study considered that psoriasis may be a disease which closely relate to autoimmunity and mediated by main of T lymphocyte, today. The course of psoriasis is intimately relate to subpopulation of T lymphocyte differentiation Disequilibrium,T lymphocyte excess activation and transduction out of control, definite gene expression abnormality. The main reason is T lymphocyte abnormity infiltration and activation. The costimulation numerator-CD80,CD86 is key factor to decide stimulation of T lymphocyte. The activation of T cell need double signals: Frist signal is specific antigen brevi-peptide which presentated by major histocompability complex-â…¡(MHC-â…¡) molecule supra Antigen presenting cell (APC) combine to T cell receptor; Second signals is B7 molecule (CD80,CD86)expressed supra APC linking to CD28/CD125(cytotoxic T lymphocyte-associate antigen-4, CTLA-4). After APC phagocytize and deal antigen(bacterium,virus or change autoantigen), to cause Antigen presenting cell (APC) of local tissue sufficient activation and enhance express B7 molecule to combine CD28/CTLA-4,through costimulation signal making immunological response take place, thus induce and maintain the pathology change of psoriasis. So costimulation passageway have main molecular organization of B7-CD28/CTLA-4 is the requisition of T cell activation and generation.We pseudo-adopt flow cytometry to detect the expression of B7 molecule (CD80,CD86)in the peripheral lymphocyte inpatients with psoriasis vulgaris and compared to normal control. In order to investigate the pathogenesis, at the same time expect to supply dependable and objective target based on for the clinical therapy of the psoriatic with psoriasis.Methods: The case group involved thirty typical ordinary psoriasis patients who were selected in out-patient clinic in the second hospital of Hebei Medical University from September 2007 to May, 2008. Select standard: A,The group have typical clinical situation of psoriasis vulgaris. B,Excluded patient with autoimmune disease such as lupus erythematosus, pemphigus, dermatomyositis, rheumatic arthritis, et al, patients with tumor, with other severe Systemic diseases or chronic diseases. C,All the cases involved had not used external glucocorticoid for two week, or taken systemic drug therapy that could effect immunological function such as immunosuppressive agent,corticoateroid hormone, et al. for one month. the case group consist of twenty four male and fifteen female, age range 7 to 56 years, the course of disease range seven days to twenty years. Meanwhile we divide all the case to two group, one is progressive stage group which have 27examples,the other is stable stage group which have 12examples.Normal control: have twenty cases . Through t-test ,sex,age has no significant deviation to the case group. they are the corresponding time period healthy volunteer who have no autoimmune disorder or family history.Adopting flow cytometry method to detect the expressed level of peripheral blood lymphocyte CD80,CD86.The specific operation according to the instruction manual of kit. The statistical treatment be carried out strictly by SPSS13.0 software and used Wilcoxon signed-rank test, p<0.05 have statistical significance. analysis the relation between the expressed level of peripheral blood lymphocyte CD80,CD86 and clinical severity degree(PASI score) by spearman rank correlation, p<0.05 have statistical significance .Result: According to the result of flow cytometry, the expression of CD80,CD86 on the lymphocytes of psoriasis was significantly higher than that in the normal control (u1=-2.394 P<0.05; u2=-4.244 P<0.01); comparing the lesions of two stage significantly higher expression of CD80 and CD86 was found in the lesions of progression stage respectively (u3=-2.130 P<0.05; u4=-2.054 P<0.05). The expression of CD80 on the lymphocytes of psoriasis with PASI score have positive correlation(rs=0.510 P<0.05), still cannot consider The expression of CD80 on the lymphocytes of psoriasis with PASI score have linear correlation(rs=0.194 P>0.05).Conclusion:1,The expression of CD80,CD86 on the lymphocytes of psoriasis vulgaris was significantly higher than that in the normal control, indicating CD80,CD86 correlation with the pathogenesis of psoriasis closely, generous expression of CD80,CD86 promote the abnormality excess activation of T lymphocyte which participate the course of psoriatic morbility.2,Comparing the lesions of two stage significantly higher expression of CD80 and CD86 was found in the lesions of progression stage respectively,indicating CD80,CD86might play a certain role in the development of disease. It offers scientific evidence for investigate the development and immunotherapy of psoriasis.3,The expression of CD80,CD86 on the lymphocytes of psoriasis vulgaris with PASI score have positive correlation, indicating constimulation molecule relation to the pasoriatic clinical severity degree, so we can evaluate the severity degree and therapeutic efficacy of psoriatic by the objective index.
