Research On The Performance Assessment And Optimization Strategies Of County Public Health Institution

Objective In the light of rural public health service system performance model, we design reasonable questionnaire for county public health institution performance assessment, find the existed problems and put forward the relevant optimization strategies to improve the service quality of county public health institution, enhance the performance of the rural public health service system, and ultimately improve people's health standard.Methods Theories and methods such as health management, biostatistics and so on are used in this research. Define the performance assessment index system through theoretical analysis and expert panel discussions, and evaluate performance of county public health institution in sample areas under the national natural fund, discuss the influential factors, and given the corresponding countermeasures. The main methods include literature research, expert panel discussion, chosen topic discussion, in-depth personal interviews, and biostatistics, such as AHP and Topsis evaluation.Result Firstly, the rural public health should perform seven items of essential functions, including prevention and control the epidemic of communicable diseases and other diseases, emergency response and handling to the outbreak of diseases and emergency public health incidents, carrying out the related laws and rules, protecting the health and ensuring the safety, multi-department collaboration and encouraging the community participation, construction and perfect of the prevention and care net at the level of the county, township and village, cost calculation and financing of public health, monitoring the health status of regions, confirming and resolving the priority public health problems of population, protection the living environment and promoting the safe using of water ,carrying out health education, promotion and encouraging health performance.Secondly, for CDC, the performance assessment index include the followings:full vaccination rates, the number of person-days of health education and environment monitoring, the incidence and timely report rates of infectious diseases, and the staff job satisfaction. For maternal and child health center, the performance assessment index include the followings: the system management rate of maternal and children, the number of person-days of health education and training, critical patient survival rate, and the maternal and infant mortality.Thirdly, the performance assessment result of the 12 county public health institution is below: for CDC, from good to bad sort is Qianjiang, Rongchang, Wulong, Gushi, Xixian and Shangcheng; For maternal and child health center, from good to bad sort is Qianjiang, Rongchang, Shangcheng, Xixian, Wulong, and Gushi.Fourthly, the factors that affect the performance of county public health institution including the followings: geography, society, government, institutions, and residents, among these factors, the government plays the most important role.Suggestion: We should not improve the performance of the county public health institution simply from its own; we should start from the harmonious society, discuss the optimization strategies from the government, socio-economic development, the institution and the residents to improve the performance of public health institution. After the investigation and analysis of the performance of the public health institution, we should start from the following aspects to improve the performance of county public health institution:Firstly, the provided public health services should suit the actual situation.Secondly, strengthen the basis of three-tied health care network in rural areas, improve the management performance of public health institution.Thirdly, accelerate the socio-economic construction and development of public health undertakings.Fourthly, reinforce the health education, raise the awareness of the health protection.Fifthly, the government financing responsibility should be strengthened, improve the investment performance.Sixthly, implement the related public health policy. Seventhly, establish the coordination mechanism of public health departments. Eighthly, reconstruction the rural public health organization system, implement the public health management functions of the government.Ninthly, strengthen the staff culture of the rural public health, improve the integral quality of the staff.
