Part one:Serum Uric Acid and Ischemic Stroke in Patients with Essential HypertensionObjective To delineate the relationship between serum uric acid and ischemic stroke in essential hypertension (EH).Methods A retrospective study was conducted in a cohort of 729 EH patients. They were classified into four groups according to serum uric acid.The odds ratio for ischemic stroke was calculated with the lowest quartile as the reference.Results (1)The prevalence of ischemic stroke in EH with quartile 4 of uric acid was higher than that in EH with quartile 1(37.91%比17.58%,p<0.05),but it was not observed in EH with quartile 2,3of uric acid. similar trend was observed in male(p=0.037),but not in female (p=0.385); (2)After the adjustment of the related risk factors , logistic multiple regression analysis showed : in EH with quartile 4 of uric acid, the odds ratio of ischemic stroke was 1.481(95%CI, 1.212-1.81 p<0.01) greater than that in EH with quartile 1, the relationship was found in male, but not found in female.Conclusion Elevated uric acid is an independent risk factor for ischemic stroke in male with EH. Part two: Renal dysfunction and Ischemic Stroke in Patients with Essential HypertensionObjective To delineate the relationship between Renal dysfunction and ischemic stroke in essential hypertension (EH).Methods A retrospective study was conducted in a cohort of 1560 EH patients. They were classified into six groups according to glomerular filtration rate (eGFR≥90ml/min.1.73m2,80-89ml/min.1.73m2,70-79ml/min. 1.73m2,60-69ml/min.1.73m2,50-59ml/min.1.73m2,<50ml/min.1.73m2 ). The odds ratio for ischemic stroke was calculated with eGFR≥90ml/min.1.73m2 as the reference.Results (1)The prevalence of ischemic stroke in EH with GFR (60-69ml /min.1.73 m2, 50-59ml/min .1.73m2, <50ml/min.1.73m2)was higher than that in EH with eGFR≥90ml/min.1.73m2(23.89%,33.39%,39.83%to17.15% p<0.05); (2)After the adjustment of the related risk factors , logistic multiple regression analysis showed : compared the EH with eGFR≥90ml/ min.1.73m2, the odds ratio of ischemic stroke in EH with eGFR(50-59ml /min.1.73m2,<50 ml/min. 1.73m2) was 1.09 (95%CI,1.08- 1.31,p =0.001),1.20(95%CI,1.08-1.33,p<0.001) respctively . eGFR decreased Per 10 ml/min.1.73m2, the odds ratio of ischemic stroke was 1.17 folds(95%CI,1.09-1.27, p<0.001).Conclusion Renal dysfunction is an independent risk factor for ischemic stroke in Patients with Essential Hypertension.
