Objective:To analyse the therapeutic efficacy and side effects between diferent inducer remission regimens for the treatment of patients with APL by retrosp-ective studies.Materials and Methods:42 newly diagnose cases with APL which were diag- nosed between May 2002 and September 2008 were collected from our hospital.The diagnostic criteria is based on internal diagnostic criteria,and 39 cases of M3a and 3 of M3b are available. There were 26 cases of male, 16 cases of female, 34.5 meta-age of onset years old, and the average age 34.55±14.19 ye-ars old. According to the diferent inducer remission regimens to calculate the complete remission (CR) rate ,the 42 cases were divided into groups with As2O3+ATRA therapy, (group A,n=12),and ATRA alone treatment, (group B, n=30) ,in order to compare the therapeutic efficacy and toxic side effects.We did the statistics analysis with x2 test,Fisher exact probabilities and multifacto-corrlation. P<0.05 has statistic significance.Results: 1.The CR rate in group A was 91.76%(11/12), respectively,in group Bwas 93.33%(28/30). There was not significant difference in the CR rates betw-een the two groups(p=1.00).2.The average time for achieving CR was 35.36±3.32 days in group A, and42.61±7.71 in group B.There was significant difference in the average time for achieving CR between the two groups(p<0.05).According to the multipleli-near regression, the average time for CR had positive correlation with the use of As2O3(β>0),and the negative correlation with infection station and the lo-wer hemoglobin level when the patients first visiting(β<0). 3. In the 42 cases, 35cases were tested with flow cytometry to chect out the immunophenotypes.The positive rate of CD33 and CD13 were 94.29%(33/35)and 91.43%(32/35).4. To study termination,the 2-year survival rate in 39 follow-up survey patients getting CR were 100% and 76.2%% in group A and group B,while the the 3-year survival rate were 100% and 61.5% at group A and group B , respectively.5.To study termination,the relapse rate was 9.09%(1/11)in group A,while 28.71%(8/28)in group B,and there was no significant differece between the ones (P=0.205).6.In the course of inducer remission, there was a significant difference of incidence rate of liver dysfunction between group A(41.67%)and group B(6.7%)(p<0.05). And there was no significant difference of incidence rate of the other adverse reactions(p>0.05).Conclution: 1.The average time for achieving CR was shorter in APL patients treated with As2O3+ATRA therapy in the inducer remission regimens.To shor- ten the time to CR can reduce the incidence of complications,such as infect- ion,bledding, and can also reduce the number of blood transfusion.At the same time,it can be shortened length of stay ,reduce costs, and improve the qualityof life.2. The remission rate in APL patients treated with As2O3+ATRA induction remission regimen were similar with those treated with ATRA only. And the survival rate of the former were higher than the latter.3. Useing of high expression of CD33, CD13 in APL,the immunophenotypeanalysis of APL may improve the accurate diagnosis rate under the unconditio-nal gene and chromosome karyotype testing .4. The incidence of liver dysfunction in the patients treated with As2O3+ATRA therapy in the inducer remission regimens were high than than those treat-ed with ATRA only.There was no significant difference . And there was no significant difference of incidence rate of the other adverse reactions , such ashe adache, muscle and joint pain, mouth ulcers, etc.
