ObjectiveThis study aim is to assessment the therapeutic effect on children with acute lymphoblastic leukemia(ALL)in our hospital,through retrospective analysis of the clinical characteristics of children with ALL admitted to the third affiliated hospital of Zhengzhou university,and analyze the factors affecting the therapeutic effect,in order to make reasonable improvement.MethodsChildren with acute lymphoblastic leukemia were collected from the Third Affiliated Hospital of Zhengzhou University from January 2008 to January 2015.A total of 418 cases were collected.Summarizing the clinical data of the first visit of the children.Following up the therapeutic effect and survival status of the children.The data obtained were analyzed statistically and compared with the data reported in China.ResultsThere are 418 patients were collected.69 patients given up treatment.349 patients were treated with chemotherapy protocol of China's recommendations for the diagnosis and treatment of childhood acute lymphoblastic leukemia(Third Revised Draft)-2006 regimen.The treatment related mortality rate and induction death rate and infection related mortality of patients treated with Recommendations for diagnosis and treatment of childhood acute lymphoblastic leukemia in China(Third Revised Draft)-2006 regimen[1],abbreviated as the 2006 protocol,was 6.0%(21/349),3.7%(13/349),4.6%(16/349)respectively.The early induction response rate was 95.8%.the relapse rate was 10.0%.The 5-year OS rates for low risk(LR),intermediate risk(IR)and high risk(HR)were(90.6±3.0)%,(84.0±4.5)%,(71.8±6.8)%.and the difference was statistically significant(P=0.024).The probability of 5-year OS rates was(87.1±2.2)%.The 5-year EFS rates for low risk(LR),intermediate risk(IR)and high risk(HR)were(89.6±3.1)%,(80.9±4.7)%,(68.9±5.8)%,and the difference was statistically significant(P=0.003).The probability of 5-year EFS rates was 81.2±2.6)%.Conclusions(1)The early treatment effect of patients in our hospital is significant,and the early induction remission rate is 95.8%;(2)The median follow-up time was 84 months,and the relapse rate of 5-years after remission was only 10.0%,which was consistent with the data reported by the domestic advanced cooperation group.(3)The 5-year EFS rate of patients in our hospital was(81.9±4.7)%,and the therapeutic effect was satisfactory.(4)The treatment-related mortality rate of children with ALL in our hospital is 6.0%,died with infection were 76.2%,died during induction treatment were 61.9%.Treatment-related complications are the main factors affecting the therapeutic effect of children with ALL in our hospital,and infection is the most common complication in the treatment,so how to prevent infection and how to control infection is an important problem to improve the therapeutic effect of children with ALL in our hospital.
