The implementation of public function is fundamental in the health inspection and supervision system. The health supervision institute of Shanghai Municipal Health Bureau was the first independent provincial health inspection agencies in China and has developed for more than ten years. This research is to evaluate the status of implementation of its public functions, to find out the weakness and the reasons of the problems, and finally to propose the improving strategies.Based on the investigation of 30 provincial-level health supervision institutes, using the general descriptive analysis, vertical and horizontal comparative analysis, this research is to evaluate the implementation of public function of health supervision institute of Shanghai Municipal Health Bureau, and to compare the status quo with eastern, middle part and western areas of the country and with health supervision institute of Beijing Municipal Health Bureau.In general, the average implementation proportion of 11 public functions of the institute is 91.5%, the average implementation degree is 90.2% and the average fulfillment degree is 83.3%. In particular, the implementation proportion of case investigation, report & complaint handling, law enforcement supervision & inspection, public health legal publicity and education, health supervision information management and staff training & management function are 100% each, health administrative licensing and health monitoring & evaluation are relatively low, with the implementation proportion 71.0% and 58.8% respectively. The average implementation degrees of items are all more than 85.0%. Functions of health administrative licensing, staff training & management and health monitoring & evaluation are much higher, with the implementation degree 95.5%, 95.3% and 92.7% respectively; report & complaint handling function is much lower, with the implementation degree 85.2%. The fulfillment degrees of these functions are more higher: staff training & management (95.3%), public health legal publicity and education (90.8%), law enforcement supervision & inspection (90.0%) and case investigation (90.0%), and the follow functions are more lower: health administrative licensing (56.2%), health monitoring & evaluation (65.8%) and health supervision & inspection (75.5%). Compared to the initiative year, the average implementation degree of 11 public functions has improved obviously. Among them, these functions have increased more obviously: law enforcement supervision & inspection (30.0%), public health emergency disposal (24.7%), health supervision information management (20.7%) and major activity health inspection (11.1%). Functions of public health legal publicity and education, staff training & management and case investigation has increased less obviously as the implementation degree of this three functions were relative high.The average implementation proportion of 11 public functions of the institute is 91.5%, far higher than the average implementation proportion of national level (67.7%) and eastern areas (70.0%), and also high than that of Beijing (77.9%). The average implementation degree of the institute is 90.2%, higher than the average implementation degree of national level (85.7%) and eastern areas (89.9%) lower than that of Beijing (90.6%).Through deep interview with the responsible persons of more than 10 departments and management departments of the institute, the main reasons why the public functions have not completely implanted are as follows: (1) Some functions such as foods and cosmetics health administration has transferred to other agencies.(2) Some of the functions are not clear, inter-departmental cross.(3)The institute is lack of enough special funds.(4) The laws and regulations are poorly maneuverable, disjointed with the system reform.(5)The institute doesn't understand the functions completely.On this basis, according to practical situations in Shanghai, this research put forward some suggestions and strategies: (1) the public functions should be clearly defined, the mechanism of the system should be straightened out. (2) The institute needs more funds, especially special funds. (3) The constructions of health laws, standards system should be strengthened. (4) The discipline construction should be strengthened in order to enhance monitoring capabilities. (5) The law enforcement conception should be changed. (6) The working procession should be straightened out, and the post-assessment should be strengthened. (7) The warning mechanisms should be established, the statistical analysis should be strengthened. (8) Change the working style and enhance the image of supervisor.
