| Objective:to detect the bleeding volume of BPH patients with different doses of preoperative usage of finasteride,to explore the best dose and course of treatment of preoperative usage of finasteride in moderate to severe BPH patients with transurethral resection of the prostate.Methods:According to the diagnostic criteria of American Urological Association(AUA),96 BPH patients were divided into 4 groups:finasteride group 68 cases,including 22 patients with 5mg(Qd po),28 patients with 10mg(Qd po) and 18 patients with 20mg(Qd po);control group 28 cases.Finasteride group and control group respectively,the average age of patients(68.2±3.8) years old and(67.5±6.5) years old,preoperative hemoglobin,respectively(134.2±3.8) g / L and(135.5±3.9) g / L,preoperative platelet,respectively(217±3)×10~9 / L and(229±8)×10~9 / L,HCT,respectively(0.42±0.02) L / L and(0.43±0.01) L / L.All patients with any of the following points are excluded;①bleeding tendency or coagulation disorder;②serious medical disease,tumors and trustable bladder;③renal dysfunction;④cystostomy;⑤resection of prostate tissue<10g;⑥preoperative usage of herbal treatment of BPH;⑦intraoperative blood pressure greater than 160 / 100 mmHg over one-third of the time;⑧postoperative pathology report showing prostate cancer or other prostate diseases.All patients were intervention for 1 week then TURP surgery,to measure postoperative bleeding volume, then compare the results of the differences between groups and correlation analysis.Results:The results showed that the finasteride group blood loss volume,the average removal of 1 g of prostate tissue bleeding volume,intraoperative prostate tissue per-minute blood loss were significantly lower than control group.This conclusion shows that preoperative high-dose application of finasteride for 1 week can effectively reduce blood loss during TURP.Conclusion:1) preoperative usage of short-term high-dose finasteride can reduce the amount of TURP bleeding.And 20mg group is obviously.(2) oral 10mg and 20mg finasteride can reduce the amount of hemoglobin of per gram of resected prostate tissue.(3) oral 20mg finasteride can reduce intraoperative prostate tissue per-minute blood loss. |
