ObjectiveThe aims of this paper are to investigate the mental health status and nutritional status of postoperative patients with gastric cancer and their influencing factors, analyze the association between psychological status, nutritional status and survival rate and quality of life of postoperative patients with gastric cancer.MethodsOur sample was consisted of 96 postoperative patients with gastric cancer in five province-level hospitals in Fuzhou city during the periods from October,2007 to June,2008.The cases were surveyed by trained interviewers with constructed questionnaires. Factor analysis was used to extracted the main target of the common factors which reflect the nutritional status of patients with gastric cancer, then calculate for each factor score and divide the level of the nutritional status. Linear trend test was used to analyze the association between psychological status, nutritional status and survival rate and quality of life of postoperative patients with gastric cancer. Logistic regression model was used to analyze the influencing factors of survival rate and the mental health status. Linear regression model was used to analyze the quantitative relationship of patients with gastric cancer between psychological status, nutritional status and quality of life.Results1.The prevalence of anxiety and depression was high among the Gastric cancer patients, with 76% and 67.7%, respectively, after 14 days by operated; with 45.3% and 26.2%, respectively, after six months by operated. There is a close line-to-line relationship between anxiety and depression. The incidence rate of malnutrition was high among the Gastric cancer patients. The low rate of protein was 51.0%, the shortage of heat was 80.2%, the limitation of haemoglobin was 53.1%, and the limitation of serum albumin was 66.7%.2. Gastric cancer postoperative patients had linear correlation between psychological status and the survival rate for six months, nutritional status and the survival rate for six months. The survival rate for six months of 96 cases of gastric cancer patients was 0.875. The influencing factors of the survival time were age(OR = 1.183,95% CI: 1.049 ~ 1.335), the stage of cancer (OR = 1.954,95% CI: 1.006 ~ 3.794) , anxiety (OR = 1.751,95% CI: 1.292 ~ 2.373), depression (OR = 4.005,95% CI: 2.330 ~ 6.886), nutritional status (OR = 0.148,95% CI: 0.045 ~ 0.486).3. Gastric cancer postoperative patients had linear correlation between psychological status and the quality of life, nutritional status and the quality of life. As a result of the quality of life variables, multivariate linear regression equation to the variables are the time of activity(X1), nutritional status(X2), yield(X3), anxiety(X4)and depression(X5). The best multivariate linear regression equation is Y = 57.889-2.420 X1-6.404 X2 +1.865 X3 +9.160 X4 +7.281 X5.4. As a result of the nutritional status variables, multivariate linear regression equation to the variables are age(X1), the stage of cancer(X2). The best multivariate linear regression equation is Y = 0.808-0.013 X1-0.242 X2.5. The influencing factors of anxiety after 14 days by operated were time (OR = 0.641,95% CI: 0.422 ~ 0.971), emotional support (OR = 0.295,95% CI: 0.167 ~ 0.519), face (OR = 0.854,95% CI: 0.760 ~ 0.960), nutritional status (OR = 0.409,95% CI: 0.228 ~ 0.734). The influencing factors of anxiety after six months by operated were yield (OR = 1.395,95% CI: 1.102 ~ 1.766), nutritional status (OR = 0.341,95% CI: 0.132 ~ 0.881).6. The influencing factors of anxiety after 14 days by operated were time (OR = 0.608,95% CI: 0.401 ~ 0.923), the main paramedic (OR = 0.423,95 % CI: 0.185 ~ 0.967), emotional support (OR = 0.286,95% CI: 0.162 ~ 0.505), face (OR = 0.761,95% CI: 0.662 ~ 0.875), yield (OR = 1.619,95% CI: 1.343 ~ 1.953), nutritional status (OR = 0.368,95% CI: 0.203 ~ 0.667). The influencing factors of anxiety after six months by operated were yield (OR = 1.429,95% CI: 1.072 ~ 1.904), nutritional status (OR = 0.455,95% CI: 0.247 ~ 0.837).ConclusionsThe prevalent rates of anxiety, depression and malnutrition were high among gastric cancer patients. postoperative psychological and nutritional status of patients are close to whose survival time and quality of life. postoperative patients should be encouraged to actively carry out the rehabilitation exercise, to employ positive coping styles and to improve their bad mood and nutritional status, which is to help to prolong their survival time, improve their quality of life; pay close attention to age and disease in patients with late stages, to improve their nutritional status; to increase the time of physical activity in patients with postoperation, to strengthen the social and family support, change Patients with negative coping styles to improve their nutritional status, in order to improve their psychological status.
